    Indiana University (IU) Health Paoli Hospital is a Critical Access Hospital in southern Indiana. IU Health Paoli serves a rural population and offers obstetrical (OB) services. The OB department offers an individualized family centered birthing setting. The hospital delivers around 130 babies annually. The mission of the OB department is to offer a personalized compassionate delivery of care and provide a quality outcome for mother and newborn. The staff on OB is all registered nurses (RN), certified in in-patient obstetrics and electronic fetal monitoring. All 14 staff members are neonatal resuscitation and advanced cardiac life support providers. 2 of the 14 RN’s are internationally board certified lactation consultants and have an 83% breastfeeding rate at discharge. Labor support offered is hydrotherapy, birthing ball, epidural and they also offer vaginal birth after cesarean. Six family practice physicians offer obstetrical services at the hospital and three of the physicians offer cesarean section services. The 2012 Annual statistics for the OB department reflected a primary cesarean rate of only 13% and no early elective deliveries prior to 39 weeks gestation. All RN’s and physicians are focused on quality, compassionate care and outcomes. All standards of care and core measures for OB care of followed and implemented to the fullest degree. The OB Nurse Director is very passionate about quality outcomes and staying patient centered with the delivery of care, decisions and policies. A special quality that all nurse’s and physicians possess on this unit is each patient, birth is a miracle.     I fear the fact of being cut open and feel that it is just so un-natural. I do however understand that it is sometimes needed to keep the baby and the mother safe, and if that was my only option I would of course not question it, however it is not something that I would choose without the doctor finding it necessary. Conditions indicating cesarean delivery include diabetes in mother; an extremely large fetus; genital herpes in the mother, which can be transmitted to the infant during a vaginal delivery; an insufficiently dilated cervix; the possibility of severe hemorrhaging by the mother; multiple births; or breech presentation of the fetus.     The government initiatives to reduce junior doctors’ hours within the NHS Plan (DH 2000) have increased the call for midwives to expand their traditional role and take on some of the tasks that in the past have mainly been carried out by junior doctors (Kings Fund 2011). Having been working within the community setting as a midwife for the past three years I was interested in extending my role in order to provide more holistic care for my caseload of clients and their babies. Holistic care for mothers, babies and their families is highly recommended within the midwifery profession and is known to provide an improved experience for women (Changing Childbirth (DOH 1993a) NMC 2012, NICE 2006). The Newborn and Infant Physical Examination (NIPE) is one element of the UK National Screening programme and is offered to all parents for their baby within 72 hours of birth and then repeated again at 6 weeks of age, usually by their GP. This role is one of the tasks that has been highlighted where midwives can expand their role (Marshall & Raynor 2010). The trust where I work has recently introduced community clinics where parents can take their baby in order to have the newborn examination performed, therefore allowing early discharge home from the unit enabling early family bonding.     All deliveries were vaginal with no health risks to the mother or newborn. The reason that some chose immediate cord clamping versus delayed cord clamping was due to unfamiliarity, not being educated about all the benefits, and apprehension of not being able to resuscitate with the cord intact. More research is needed to provide EBP research to the hospitals and staff to educate them on the benefits of delayed cord clamping to the newborn compared to immediate clamping of the cord. The research done will help provide evidence that resuscitation can be achieved with delayed cord clamping. More research needs to be conducted so proper time of delayed cord clamping can be recorded. This also allows a set system of guidelines that are the same and the procedure followed would be strategic. EBP research has shown that delayed cord clamping can be done on pre-term, term babies, and babies that may need resuscitated. The delivery rooms can be set up and ready in case resuscitation or phototherapy is needed. If a provider does not want to wait due to resuscitation or other issues, milking the cord can provide the neonate with the extra blood from the cord in a shorter amount of time.
