N And Her Family: A Case Study
N and her family were categorised as family support and I acted in a supportive capacity role while assessing the needs and risks. I adopted a person centred approach derived from Rodgers (1959) as agreed in supervision. This is in keeping with relationship based social work highlighted by Jowett and Spray (2012) and involved working in partnership alongside N by asking what support she needed (Tosone, 2013). Perry et al (2010) agree it can enable service users to communicate what is important to them. This is in line with the Department of Health (2016) outlining the need for co-production between providers and service users to transform services. Throughout all my discussions with N her aim was to integrate D back within the home as she identified

As far as I could think, Sone and her family was an industrious family. The story begin by telling how Sone’s father managed to keep his business. Moreover, in their temporary camp, there seem to be something that kept the family busy. For example, Sone’s father were fixing the place where they lived and Sone was working in Personnel Department that she had to go back and forth between Area A and D. When her father was asked to give up any given dangerous instruments or weapon, he said that he need those tools for work. This story also tell how the family was hopeful. When they were in the temporary camp, they still hold strong to their beliefs. It said that the problem that they faced on that moment was only the beginning. Also, every Sunday…

Not always having the final say in a social work position can be challenging when we see how implementation is carried out by caregivers on a first hand basis. The study by Wilber and Mosqueda was important in realizing that although many facilities were on board with the idea of person centered care, there were ultimately setbacks within each agency itself. I have seen this in my work at Meadowlark as we work towards a cohesive work environment with each caregiver and staff member providing the same level of care. Sometimes, both internal and external forces can make the managerial side of healthcare difficult as Meadowlark sets standards and moves towards new strategies and models of care. It’s important to keep in mind where we can improve as a healthcare facility, knowing that we are continuing to stay involved in the research pertaining to our residents and their care.…

Different approaches to care planning and assessment can lead to fragmentation of care. I have received referrals for clients wanting to change providers due to their assessed needs not being met or assessed and a care plan implemented not right for the individual. These assessments were not person centred led but the ideas of the assessor and in some cases the family. This is still happening due to families who want what they think is right and their own personal views for their family member. A couple of the service users referred for assessment and change of service came from an old style of care where they stayed in institutions and residential services, where person centred care was not implemented. The care system has moved on and improved the way people are cared for but some providers and professionals are slower moving forward with the improved ideas and way of working.…

No two service users are the same, therefore each service user is to be treated as an individual. Identifying need is important in health and social care because health care professional must know what they are aiming towards whilst caring for service users. Therefore, identifying their needs is the first step into providing the best care for them. An example of some research that identifies need is The Family Nurse Workforce: A Study for the Family Nurse Partnership National Unit. This research is important because it gives information about what the Family Nurse Partnership programme is, and what they do, which is to provide protection and support for vulnerable young first time mothers. The service offers home visits carried out by specially trained nurses. The Family Nurses and supervisors are required to provide all their qualifications and background to ensure that they have the proper training e.g. a knowledge of various medical conditions and the services available to help cope with this condition.…

The assignment is intended to discuss a case I worked on whilst on my final placement and use an analytic stance to discuss the interaction and interventions used with the service user. In order to maintain confidentiality and anonymity of the service users in this case study I will be using pseudonyms for all people involved and mentioned in relation to this case. The assignment will demonstrate professional judgement, accountability of a social worker and the statutory requirements of protection and intervention when working with vulnerable adults. The case study will focus on my work with Sarah throughout my time on placement and assess my principal interventions as well as locating my work within the wider discourses around risk deploying both recovery and exchange models. It will also highlight the strengths and limitations within the process of recovery. Within this case study I will aim to describe and analyse the practice, theories and values that have emerged by carrying out this piece of work. The assignment will critically evaluate my practice with specific reference to the social work skills used. By reflecting on my practice I hope to demonstrate how I have considered issues such as power imbalances and social injustice and how this consideration has assisted me to work in an anti-oppressive, non-judgemental way with the service user involved. With reference to this particular case study I will also demonstrate to the reader the power imbalance that existed between the professionals in terms of their approach. This was clearly evident in relation to the social model approach versus the medical model approach. Drawing on my observation and the experience of working within an inter-professional team I will provide critical appraisal of the issues and barriers that existed. I will also compare, contrast and evaluate the medical and social model of mental health and mental disorder by referring to appropriate literature and…
