Needs Assessment Guidelines A. Identification of the Setting – Identify the setting in which your program will take place. Be specific in describing the setting by including as much information as possible.

c. Identify three primary health outcomes of interest. These are based on your perceptions of the conditions affecting your population. As your needs assessment progresses, you will choose a specific health outcome to address. 1. Weight Management/Weight Loss 2. Increase consumption of low fat food items, vegetables, fruit, and whole grains. 3. Increase physical activity 4. Increase administrative support of healthy alternatives

d. Write a comprehensive summary description of the setting. Identify key characteristics, traits, and behaviors of the community, school, etc. This can be based on your perspective of the setting. Be sure to identify why you think these characteristics impact the health outcomes of interest.

The population and the setting are the employees at Mississippi Care Center of Alcorn County, which is a Long Term Care Facility. MSCC is located in Corinth, MS which is located in the very northeast corner of the state. As of the 2000 census, there were 14,054 people, 6,220 households, and 3,800 families residing in the city (Corinth, Mississippi, 2012). The median income for a household in the city was $23,436 and the median income for a family was $35,232 (Corinth, Mississippi, 2012).
