Observation In LF Room
Ken was in LF room lying in his bed @ 1925.

Staff was getting him up to have him redirected to his own room when the other resident was observed by 2 staff to strike Ken on the back of the neck with his cane.

Incident documentation in witness report.

Seperated resident. HIR initiated, cold pack to back of neck. 911 called, instruction for care to have resident rest. PERRL 3+ , Hold his head still, he sat in the chair he was having nourishment to distract him. BP 140/75 P 60 resp 18 sp02 97%

Skin tear on his right forearm sustained. It was bloody and cleansed with saline and tegaderm applied. Complained of pain in the back of his neck, reddened with 2 pinpoint open areas as well as swelling. Family notified Ken Moore jr. He was satisfied with medical and nursing interventions. Transferred to hospital with spinal precautions. Administrator and DON notified.

LF was yelling at staff and Ken I went into the room and went to help staff remove Ken from the bed. As I was coming in the room, Lawrence picked up his cane and with both hands on the cane, raising it high into the air and using his full ability struck Ken from behind. As he went to strike again I grabbed the cane from Lawrence to stop
