Obstetric Case Summary
The patient is a 92-year-old gentleman who presented to the emergency room after having had a fall a week prior to this presentation at which time he was seen in St. Joseph's Wayne Hospital which he was found to have a comminuted proximal humeral fracture placed in a sling and discharged home. The patient however is not feeling well. He is having increasing pain and he brought in to the the emergency room by a friend. His medical history is significant for hypertension, atrial fibrillation on I believe he is on Coumadin. He also appears to be on Metformin. In the ED he required multiple doses of Morphine Sulfate in order to control his pain. He is also found to be in afib with a rate of 101. He is admitted inpatient to the telemetry floor and subsequently developed a very rapid afib and was seen by …show more content…

I believe based on the complicated history, the fall previous still requiring a significant amount of pain medication, the home sitituatuin which is not conducive to him at this point receiving any type of rehabilitation this patient deserves acute inpatient hospitalization and a short stay in a subacute to get him back to baseline to function and assist with the care of his

    Detailed description of event including timeline: Thursday 3:30 pm Mr. B a 67 year old patient was admitted to the ER after a tripping and falling over his dog at his home by nurse J. He was complaining of 10/10 pain to his left leg and hip region but appears to be in only moderate distress. Mr. B’s vital signs were stable at time of admit with a blood pressure of 120/80, heart rate of 88 and increased respirations of 32. Left leg is shortened, swollen in the calf, bruising present and limited range of motion to left leg. Dr. T looked over data and ordered Mr. B to have medications for pain control and sedation in order to perform a manual manipulation in realigning Mr. B’s dislocated hip. 4:05 nurse J administered IV Diazepam per Dr. T’s orders. With no affect Dr. T. orders Nurse J to give 2mg IVP Dilaudid at 4:15. 4:20 Dr. T. orders nurse J to give 2mg IVP Dilaudid for muscular skeletal relaxation. Dr. T. notes that per patient’s weight and regular usage of Oxycodone, it was making it more difficult to achieve the level of sedation required for manual manipulation. 4:25 Mr. B appears sedated and comfortable and procedure of manual manipulating the dislocated hip back in place goes successfully and concludes at 4:30 with the patient resting and no signs of distress. Nurse J. places Mr. B on an automatic blood pressure machine and oxygen saturation to go off every five minutes and leaves to take care of another patient, with Mr. B’s son at the bedside. 4:35 Mr. B’s blood pressure is 110/62 and saturation of 92% on room air.     the Ninth Circuit considered the promotion of health in the undue burden clause by stating that the bigger the burden was, the stronger the law’s justification for women’s health must be. The Seventh Circuit used the same approach in Planned Parenthood v. Wisconsin, Inc. v. Van Hollen (2013) when they stated that the burden created by a law must be justified by a state’s interest in promoting health as well as by medical evidence backing the necessity of the law. Hellerstedt claims that Fifth Circuit was right in not considering the state’s interest and how it balances with the regulations of HB 2 (Shimabukuro 2016). Utilizing the balance between states’ interest and severity of the regulation is what the Supreme Court has to do to protect Casey’s intentions of preserving Roe. The court that decided Casey intended to balance the states’ interest of protecting mothers and viable fetuses with a woman’s constitutional right to an abortion (Shapiro 1995).
