Student On-the-Job Training Waiver

Dear Sir/Madam:
We would like to inform you that permission is hereby granted to our son Edgardo E. Cambel Jr. to participate in the On-the-Job Training Program that is a requirement in the course BS Electronics Engineering, under the College of Engineering. We understand that the said skills training will cover 240 training hours, which will be conducted in REMEC Broadband Wireless International Inc., located at Carmelray Industry Park I Canlubang, Calamba City Laguna, from April, 2014.
We are aware that the Teacher-in-Charge will conduct visitation (if possible) and monitor the performance of the student, and the immediate superior within the company will take all the necessary precautions to keep the student safe.
However, if despite all efforts taken, untoward incidents beyond their control occur, we will not hold Laguna State Polytechnic University-Sta.Cruz Campus and the company where our son took On-the-Job Training liable.
