1. Personal Particulars
The following details should be entered into this section: • Foreign Service number • Rank or Title • Family name or Surname • First given name or Christian name • Second given name or Middle name • Third given name (if applicable) • Preferred name (the name that you prefer to be called)
2. Contact Details
Enter your current contact details into this section. If your mailing address is the same as for your home address, write ‘As Above’ in the mailing address section. Ensure that the e-mail address indicated is correct as this is the preferred method for correspondence.
3. Other Details
The following details should be entered into this section: • Gender (place a cross in the correct box) • Marital Status (place a cross in the correct box) • Date of Birth (as shown on your Birth Certificate) • Birthplace (as shown on your Birth Certificate) • Country of Birth (Select Country from list) • Religion • First Spoken Language (Select from list) • Second Language Spoken (If applicable)
4. Dependant Details
Enter your dependant details. This information may be used later in the process for immigration purposes. Attach additional details on separate sheet if insufficient space.
Part B – Corps Preference
Select the Corps that you wish to transfer to by placing a cross in the corresponding box. If known, also enter the specialisation or trade you wish to be employed in.
Part C – Previous Military Service
The following details should be entered into this section: • Name of Military Service (e.g. Australian Army) • Country • Substantive rank (full rank not temporary) • Date of last promotion • Date of Enlistment (date of initial entry to the military)
