POSHO at BDSC met with Mr. Mohammad Mia on March 2, 2016 at 20:10 hours to find more about the possible causes that could have contributed to the fall occurred on January 20, 2016 in the kitchen. To the POSHO’s question on the condition of the floor at the time of the mishap, he stated that the floor was neither dry nor wet. Apparently, he meant to state that the floor was moped 5 to 10 minutes prior to his fall. However, according to the night shift supervisor, 6:00 am period is a very busy time with multiple activities all occurring at the same time in the kitchen it is the time when the dining facility is the busiest.
