becoming independent again and returning to his normal baseline ADLs. The patient reports that he does not have a walker or cane at home, has steps outside to get into his house and stairs in his two-story home, and a bathtub to shower in with no chair. The patient wishes to not use the walker at home, but a cane because it will be easier to maneuver around the house.
The patient and I determined going to rehabilitation or transitional care unit (TCL) in Memorial would help gain his independence. The patient acknowledges that he may not be the same as he was before, but he wants to be as independent as possible by being able to climb the stairs, walk with a cane, bath himself independent, and going for walks again, even if they are shorter. The patient will include his wife in his care plan so together, they can learn to compensate for things that require help such as walking.
