Development of web-based Patients Information System for Barangay Tejeros Health Center
Main Problem: A lot of time are consuming in recording patients information, researching patients name and securing the files.
Specific Problems: 1. Manually recording patient’s information takes a long period of time. 2. Time consuming on searching patients name through their book record. 3. File manageability. 4. Privacy of the files are being exposed. 5. Inaccuracy of data records.
Causes of the Problems: 1. File cabinets are always unlocked. 2. Some patients forget their control number. 3. Some records are not organized.
Effects of the Problems: 1. Resulting in inaccurate records. 2. Unauthorized person may have the data or a copy of data.

II. Definition of the Topic:
A. Importance and Significance
The purpose of this study is to implement an information system for the patients of Barangay Tejeros Health Center.
Our motivation for conducting this system is to help Barangay Tejeros Health Center on their files to be more organized, lessen the time consumed and secure patients information.
B. Manageability:
What:
The patients information system is a system that allows Barangay Tejeros Health Center to have an organized and secured information while conducting medical check-ups of patients.
Who:
The users and beneficiaries of this system will be the Medical staffs, Doctors, and patients of Tejeros Health Center.
Where:
The system will be used for everyday transaction of Tejeros health center.
When:
Tejeros health center has its everyday transaction. It is organized by the medical staffs and doctor. They are using manual recording that causes time consuming, unorganized and unsecure files that happens
