AFFIRMATION OF WAIVER OF RIGHTS and QUITCLAIM

We, ______________ and ______________ , husband and wife, Filipino and residents of _______________________, ____________________, after having been duly sworn to in accordance with law hereby depose and state thus:

1. That we are the parents/guardian of _____________________________, who is a bonafide student of AMA Computer Learning Center (ACLC), Butuan City under Information Technology (IT) Department;

2. That our son/daughter has applied for an On the Job Training (OJT) at ______________________________, as part of his/her academic requirement;

3. That _________________, which is located at ______________________________ obliges herself to issue a certificate of completion at the end of his/her OJT based on the following conditions:

a. That our son/daughter must perform the task well that are assigned to him/her;
b. That our son/daughter needs to complete a certain number of hours of duty as prescribed being part of his academic requirement;
c. That our son/daughter is bound to comply the house rules of _________________ or its firm’s policy during the entire duration of his/her On the Job Training (OJT);

4. That we understand at the start that the presence of our son/daughter in _______________ is not motivated by an Employer/ employee relationship;

5. That we voluntarily agree that AMA Computer Learning Center (ACLC) and _______________________ , would not be in anyway liable in the event that our son/daughter incurs liability in relation to his/her OJT;

6. That it is by virtue of these presents for ourselves, our heirs, executors, assigns and administrators that we hereby REMISE, RELEASE and FOREVER DISCHARGE and forever WAIVED to AMA COMPUTER LEARNING CENTER (ACLC) and ___________________________ from any or all forms of actions, reckoning, promise, claims,
