This is a followup dermatology clinic visit. See Dr. Tara Stewart's dermatology consult from 05/13/2016. The patient has posterior hip herpetic neuralgia over his right neck continues to be troublesome for him. He says his other issues that were discussed in May are much improved. These consisted of apparently some scales and scaly macules and patches over his chest, arms, shoulders, neck, and ears that had some crusting. This he says is all improved and doing well. He is currently followed in the Pain Clinic for his post herpetic neuralgia. His cutaneous issues improved about one month after the dermatology clinic visit in 05/2016. He is now concerned about residual pain in his right scalp and neck and one itchy site on his right thigh that comes…

Those in the healthcare profession are respected, hardworking individuals who went above and beyond to prove themselves capable of helping others in the greatest way possible. My career goals are very straightforward. I’ve taken several steps in high school to accomplish these goals by being well disciplined in my studies, and participating in a Health Careers Internship Program. This program offers me a variety of opportunities that make me a more applicable in a healthcare setting. I’ve learned how to build a strong résumé and cover letter, and how do professional interviews well. We also get the chance to observe and participate in a medical environment. I’ll be shadowing with Dr. Steve Carr at Northern Star Therapy and learn the basics of physical therapy. I’ve done also done volunteer work in a Geriatric setting, at the St. Benedict nursing home in St. Cloud. My plans following high school are to attend the University of Minnesota in the College of Biological Sciences. My hopes are to complete my undergraduate in there in the Pre-Medicine program, attend graduate school, and specialize in a currently undecided field of…
The current position I hold is in infection control at a large facility of approximately 350 beds. It has been an interesting journey in which I have grown beyond anything I could have imagined. It is a difficult position filled with challenges. My goal is to use the Masters in Public Health in my current role. The program I will be leading at work in January will be in need of renewal. I have hopes that participating in the Walden MPH Program will move me forward and give me the tools needed to build a vibrant infection control program.…
In addition, my employment experience has provided me with great respect for social and ethical views that differ from my own. This position has given me the opportunity to decide if a career in medicine is truly where I would like to be. Working at the hospital has been a truly challenging experience through dealing with the magnitude of death, illness, and recovery. This experience has made me realize that a career in medicine is where I need to be. I have experienced a multitude of emotions and in the end I have realized that the reward of knowing that a life was saved or that through the death of a patient, we have made groundbreaking discoveries in medicine is far greater than the emotional tensity associated with the experience.…
There are two instances in my life that has inspired me to gain interest in medicine. My grandmother when I was around the age of twelve received double bypass surgery and I fell in love with the physician who took care of her. The physician was a middle aged woman who was one of the kindest person I have ever met, and she gave us the news of how everything went fine during surgery. She also gave us great confidence in how my grandmother would return to normal soon. The look on my parents and aunts face when she gave the news gave me goosebumps and to this day I feel the same goosebumps when I think about this. I just wanted to one day be in a position to deliver good news to a family and to see that “look” of relief on their faces. Another…
I than was originally inspired by a nurse practitioner that I currently work with at a nonprofit company that serves the economically disadvantaged population. I was amazed at the quality of care she provided regardless of pay. This is the moment I knew I had to move forward and pursue my dream.…

I know that there are limited spots, but am willing to prove myself. I know that it will not be easy, but that is a risk I am willing to take to achieve my goals.…

One of my electives that I had chosen to pursue for three years of my high school career was the health services program. Throughout those three years, I have gained knowledge about medical terminology, various types of medical careers, and participated in many community service activities. While learning about the different health professions, I found my own passion to want to become a clinical dietitian. I have always been intrigued by promoting health and wellness. During days I would be free, I would research the web for information about gluten free-foods, vegan…

Everything in my life began to change, from my friends to my attitude. My future was looking bright. The only thing left was to pick a profession, something that would ignite my spark. That was when my parents were involved in a terrible accident. In March 2009, my parents were pedestrians struck by a car. My mother was diagnosed with a fractured tibial plateau, and was wheelchair bound for five months. During this difficult time I accompanied her to numerous appointments and met many different medical personnel. Surprisingly, I became inspired. It was an awful time for my family, but it was an experience that changed my life. Everything about the healthcare system fascinated me. I asked many questions about medications, imaging techniques that were used on my mother, and job descriptions of practically everyone I had encountered. I remember telling a nurse about how I found the hospital to be the most interesting place that I have ever visited and she replied, “Well if you feel that way, you should become a doctor, they practically live…
