I'd also add to check her bladder for distention which can affecting her bleeding and location of the fundus.

Nursing interventions for this post partum patient would be:
◦VS every 30 minutes
◦document the amount of blood loss, color and odor
◦document that you were unable to palpate fundus
◦document and visually check the peri area
◦help patient up to void and document her output
◦help patient back to bed and place a new peri pad in
◦palpate the fundus again
◦If unable to find fundus again call the doctor to report your findings. At this point the doctor may order oxytocin which will make the uterus contract to help stop the bleeding.
◦Continue to monitor

Great bullet points - very simple and easy to follow! I believe vitals are taken q15 minutes for the first hour after birth.
