How to File the Form:
The original form must be filed with the court. To file the form, you may do one of the following: (1) Mail the form to: Volusia County Courthouse, 101 N Alabama Ave, DeLand, FL 32724 (2) Bring the form to the court in person (please check court hours before coming to court) If you file the form by mail, you will be notified by mail of your new court date. If you file the form in person, you may be immediately scheduled for court if there is time available on the court calendar or you may be given a new court date. A copy of the form must be filed with the City Attorney’s Office – Ordinance Enforcement Division in person (please check the City Attorney’s office hours before you go in person).

Within 30 days of your initial appearance in court
