of practice of mental health nurse specialist by using NMBA competencies
The professional attributes are the significant element for psychiatric nurse specialist to become a good nurse .It is defined as “a characteristics, which underpin competent performance “(NMBA, 2006). One of the main professional attributes that enhanced the practice of psychiatric nurse specialist is honesty. “Honesty includes keep promises and maintaining confidentiality”(McCabe, 2014). “Many clients with psychotic disorder and emotional cannot trust easily to others because they may have lied to or hurt in the past and this make difficult for them to trust again” (McCabe & Sambrook, 2014). Psychiatrist nurse specialist are always being honest with the clients when providing care to them because if the patients cannot build trust with the psychiatrist nurse who provides care it will hinder the patient care. According to NMBA (2006) it is important for nurses to maintain confidentiality of a patient and tell them the truth about their ongoing care. Secondly, being there for the patient is also a complex attribute that is also referring to as “nurse presence” (Martin & Street, 2003). Being there for patient includes spending time with the patient and listening to them that helps the patients to express their feeling and it will reduce the fear of stigmatization and social exclusion (Martin, 2003). So “in order to be there for the patient psychiatric nurse specialist is willing to invest time in the patient it develops the therapeutic relationship between clients and nurses” (Martin, 2003). In contrast, Moyle (2003) explain “the patient with depression felt that the presence of psychiatric nurse specialist in particular bedside presence is beneficial to the relationship as the presence of psychiatric nurse helped to alleviate their fear”(Moyle, 2003)
Next, ability to value and support diversity and maintain individuality is also essential professional attribute. “Psychiatric health nurses have the ability to influence individuals and groups to achieve positive outcomes of culturally competent care for diverse population”(Berg, & Hallberg, 2000). They encourage the patient’s independence by offering suggestions without taking control (Berg, & Hallberg, 2000). According to NMBA (2006) nurses have to accept individuals regardless of race and culture without imposing personal values and attitudes on another. Further, understanding and self-awareness is also an important professional characteristic. Understanding is important for both patients and nurses. Johannson & Eklund (2003) determined that “when psychiatric nurse give proper attention and appeared interested patients felt understood” (johannson & Eklund, 2003). Further, self-awareness is an important attribute to develop therapeutic relationship in psychiatric health nursing. Psychiatric nurse behaviourally manifests self-awareness. Psychiatrist nurse specialist need to be self-aware because they deal with different patients having different backgrounds, thoughts and beliefs. Without self-awareness it is hard to maintain therapeutic relationship with patients. O’Brien (2001) “all psychiatric nurses identify that their personalities could affect the way they responds to their patients and that had to be self aware to know how to approach interactions with different patients”. So it is an important attribute for psychiatric nurse specialist to be requiring to practice within ethical and professional framework and scope of practice (NMBA, 2006).
Scope of practice is established by legislation and professional’s standards, code of ethics (Hogan, 2012).
According to Nurses and Midwifery board of Australia (2007)“A profession’s scope of practice is the full spectrum of roles, functions, responsibilities, activities and decision-making capacity that individuals within that profession are educated, competent and authorized to perform and is determined by the profession itself “(NMBA 2007). It is important for every healthcare professional to work within their scope of practice to provide safe and quality of care to the patients (Berman.et.al, 2010). In Australia, “the scope of registered nurse is outlined by regulatory requirements of the Nursing and Midwifery board of Australia and the scope of practice of all mental health nurses in Australia are guided by ACMHN scope of practice of mental health nurses” (Ruth, Katie & Debra, 2012). In relation to education psychiatric nurse specialist registered nurse require minimum of master’s degree in psychiatric nursing and some institutions require certification by national credentialing association to work as a specialist (Jcob, 2009). In contrast, psychiatric-mental health nursing is responsible for functioning within the limits of their education and training. On the basis of competency standards psychiatric Mental Health Nurse specialist can provide primary mental health care to patients seeking mental health services in a wide range of delivery settings. (National standards of mental health nursing,
2005).
Psychiatric nurse specialist performs same functions of psychiatric nurse who work at basic level. They maintained health and promote health including health assessments, health teaching, preventing potential complication (Basavanthappa, 2007). Moreover the nurses supports “the level of functioning to encourage self -efficacy case management activities including monitoring of client’s functional level and medication effectiveness, problem solving and supportive counseling, care planning and referral to community resources”(Basavanthappa, 2007). Further, Nurse specialist can guide direct and strengthens the client’s progress towards independent functioning health teaching of client and family in relation to promoting mental health (Hogan, 2012). Pscyhiatric nurse who have “achieved additional education can do consultation liaison activities in which they promote optimal mental functioning and they also work as a educator with other nurses and health caregivers and they can also work indirectly as a consultant with in their scope of practice”(Jacob, 2009). Providing psychobiological interventions to patients such as teaching, administrating and evaluating medication are also within the scope of psychiatric nurse specialist (Jacob, 2009). Lastly, Psychiatrist nurse specialist also assist the patients with electro conclusive therapy and other somatic therapies under their scope of practice (Basavanthappa, 2007)
In conclusion, it is concluded that professional attributes and scope of practice is a core element of pscyhiatric nursing practice. Professional attribute such as being honest, being their for patients, demonstrating self awareness and understanding are essential characteristics of psychiatric nurse specialist which help them in developing therapeutic relationship with patients and providing quality of care to them. Further, to work with in scope of practice is important for nurses to provide safe care to patients and maintain standards of care that are set by state legislation.
