Pros And Cons Of Managed Care Organizations (MCO)
My main point was to focus on the cons of Managed Care Organizations (MCO) and their impact on how and where nursing care is provided, after the implementation of the Affordable Care Act (ACA). Potter and Perry (2017) claim that the ACA will force more nurses to provide service in community-based care settings, potentially required to relocate or begin jobs in order to “practice in community care centers, schools, and senior centers” (p. 7). With the ACA, it is now the requirement of a nurse to overcome many challenges in order to provide “patients with the best-quality care in an efficient and economically sound manner” (Potter & Perry, 2017, p. 7). As health care cost continue to rise, nurses must find new ways to adequately utilize health care and patient resources in a wise manner, different than has ever been done before (Potter & Perry, 2017, p. 7).

Potter & Perry (2017) seek to answer some of these questions regarding cost of care. Hospitals who provide services for patients with Medicare and Medicaid “receive a set dollar amount for each patient based on the assigned diagnosis-related groups, regardless of the patient’s length of stay or use of services” (p. 15). Therefore, in order to compensate for receiving less money for certain patients care costs, other citizens within the community, not using Medicare or Medicaid, are seeing an increase in the costs for the care services they are provided with when needed. It is the goal of many states to try and stop care costs from increasing, in an attempt to return these increased costs to the lower amount that they were prior to the

    Patients that have Medicaid have poorer access to care and poorer health results than patients with private insurance. In this case, privately health insured patients are able to discover more opportunistic treatment options, swifter medical care and are able to access more primary or preventative care. Medicaid patients have a difficult time accessing health care from medical facilities due to the poor reimbursement for provided health services. “Medicaid typically pays physicians 56 percent of the amount that private insurers pay” (Dayaratna, 2012, p.3). As doctors continue turning away Medicaid patients, it will remain difficult for these patients to seek health care from primary or specialty doctors. As a result of Medicaid patients being turned away, they are subject to late diagnoses, which put them at an increased risk of serious health conditions or even death.     In 2010, the United States healthcare system began its dramatic transformation as the president signed comprehensive healthcare legislation into law. The legislation, cumulatively referred to as the Affordable Care Act (ACA), offered 32 million, previously uninsured Americans, access to affordable health insurance (IOM, 2011). As Americans rejoiced and signed up for insurance, concern mounted as to whether the current healthcare system could tolerate the increased patient load. The nursing profession, in particular, was expected to face several obstacles in response to a transformed healthcare system (Trouble, 2014). According to an article in Health Affairs, addressing these obstacles requires effective production and use of the nursing workforce (Rother & Lavizzo-Mourey, 2009). In an effort to guide the impending increase of Americans expected to seek healthcare, the Institute of Medicine (IOM) along with the Committee on the Robert Wood Foundation (RWJF) formed an initiative on the future of nursing. The 2-year initiative served as an action-based model for the restructuring of the nursing profession (IOM, 2011). The committee identified four key messages that guided the IOM discussion including the future of nursing education, nursing practice and the nurse’s role as a leader.
