Johns Model of Structured Reflection (Johns 1998)

1.Write a description of the experience. Ask yourself:

*'What are the significant issues I need to pay attention to?'

I was asked to complete a pre-operative assessment on an elderly lady for cataract extraction, using an I.C.P. for intra ocular surgery under LA. During the assessment process she expressed several times that she was extremely sensitive to things going near to her eyes, also during the assessment I observed from her body language and tone that she was very nervous and anxious about the whole being in hospital experience. Throughout the assessment I tried calm her down and gain her trust by being friendly and humorous but at the same time explaining every step of the procedure and what she was to expect by the end of my part of the assessment she had relaxed considerably. As I am not qualified to complete the assessment I had to get one of the senior-nurses to come and complete the assessment by taking all the measurements and readings from the eye all of which are vital before the commencing of the operation at a later date. The first test is to measure the eye using a slit lamp. I had explained the test to the patient and said there was nothing to worry about, as it was a simple routine procedure. The lady felt quite confident about completing the examination without to much fuss.

I then left her in the examination room whilst I went to find a staff-nurse. When I returned the senior-nurse walked in and introduced herself to the patient and proceeded to start the examination, which initials the patient to sit forward in a headrest and to stay very still and not blink or move the eye. This the patient found very difficult to do and kept blinking. In response to this the senior-nurse asked her to sit still and stair straight ahead repeatedly, the patient did not respond well to this and became more agitated and started to

I asked how she is and she smiled which means she is fine as she has difficulty speaking. I asked her if she would like to have her bed bath and she said "yes please". I informed her that I also need to carry out a risk assessment for skin breakdown to identify if she is at risk of developing a pressure sore. I explained the procedures that she will expect, the reasons behind these and I asked for her permission to carry on. She obliged by softly saying "ok" in a very low voice.…

In my role as a specialist nurse I am involved with caring for patients within a community setting and providing a holistic assessment of their needs. This can involve assessing patients as a result of a supported discharge from secondary care or referral from General Practitioners (GP’s) for issues related primarily to their cardiac condition, their general overall health and any other health related issues. These assessments can take place within the patient’s home or within a clinic setting. These assessments are quite commonly carried out with very little prior knowledge regarding the patient and this is currently an area under review. For the purpose of this case study I will look at a lady who attended a clinic session within a GP’s practice.…

An elderly lady, 78 year old Mrs Jones was admitted to the unit from a local nursing home following an acute myocardial infarction. In order to gain in my clinical skills experience I was asked to accompany and observe the staff nurse who was to carry out the catheterisation. The nurse told Mrs Jones that she was just going to pop a catheter in. There were no explanatory details towards Mrs Jones about what the procedure precisely entailed, and she was not informed of the risks or benefits. Therefore Mrs Jones was unable to ask any questions, or express any fears or anxieties. On commencement of the catheterisation Mrs Jones was quite clearly very distressed by what was happening to her. She was lashing out at the staff nurse, shouting "no get off me", and with great force tried to keep her own legs shut, but the nurse continued to proceed until after several attempts the catheter was in place. This was a procedure that was carried out without the patient 's consent.…

My clinical instructor provided right supervision for me while I administered medication to my patient. My instructor was in the room when I was administering the medication and assessing the patient. The patient was very cooperative and gave permission for me, a student nurse, to provide clinical nursing care at the beginning of the shift, and again while I was performing the assessment and preparing to administer the an IM injection. I discussed with the patient that I have administered this medication before and described in a clear concise manner of what the process entails. In accordance to the five rights of delegation (right task, right circumstances, right person, right direction/communication, right supervision) the delegated care was appropriate. After five minutes the patient’s blood pressure dropped from 155/100 to 130/90 and she rated her pain 5out of 10, on a scale from 1-10... Within the next five minutes her blood pressure dropped down to 121/80 and rating her pain 3out of 10. The last five minutes her blood pressure was 90/66 and she claimed mild discomfort, no…

The reason why I choose John’s model of reflection as it has a structured of reflection that is to increase the awareness of how I think and act upon to improve the practice in my unit. John’s model (2000), structured of reflection can be used as a guide for analysis of a critical incident or a general reflection on an experience. I prefer to use John’s model structured of reflection because John supports the need for the learner to work with a supervisor throughout their learning experience. Where he refers to this as guide reflection and recommends that nurses use a structured diary. According to Heath (1998), John’s model has been found to be attractive to be used because of the cue questions that allow an individual to expand their thinking.…

Wheatley (2006) recommended that we view our approach on leadership with a set of new eyes. By this Wheatley meant that we changed our view on leadership and change our linear and organized views and follow that of the disciplines of physics, biology and chemistry for example. This change in leadership view would focus on a free flowing, open-space, anything can happen philosophy. Wheatley believed we should have many leaders and for leaders to form connections with nonprofessional staff members.…
