Consider your workplace as a learning environment and reflect upon three areas for improvement.
The term reflection refers to reviewing experience from practice so that it may be described, analysed, evaluated and consequently used to inform and change future practice.” According to Somerville and Keeling (2004) nursing is a profession where reflection is important whereby it helps to identify strengths and weaknesses. Jarvis (1992) mentioned that reflective practice can also be known as a learning process through and from the experiences towards gaining new ideas.
The two type of reflective practice by Schon (1987) are the capacity in reflecting in action and on reflecting on action. Based on Schon (1987) theory reflection in action is a process which allows us to reshape what we are working on, while we are working on it. On the other hand, reflecting on action and thinking back on what we have done in order to discover how our knowing in action may have contributed to an unexpected outcome.
There are many types of reflective theory to reflect on action. Gibbs Model of Reflection is a common model of reflection that is used within the health professions. It consists of 6 stages which are descriptive, feelings, evaluation, analysis, conclusion and action plan.

Description
Since graduation till date, I have been working in multidisciplinary ward in a private hospital in southern of Malaysia. I was the team leader for that day and upon taking over report, I felt anxious and worried as I have to work with two junior staff nurse and hospital aide on that particular day. There were a total of 20 patients on that day. Two patients were planned for operation and one patient was to be discharged and three patients were planned for admission. At 1700H, I received call from the operation theatre (OT) and therefore I requested junior nurse J to accompany me to fetch the patient back to ward. Upon arrival to the ward, I instructed nurse J to handle the patient

    In this reflection, I have utilized Gibbs (1988) Reflective cycle. This cyclical model is a recognized framework to assist with a critical reflection on practice. I chose to use this cycle because it will improve my nursing practice continuously, and learning from the experience for better and improved practice. Gibbs (1988) reflective model consists of six stages. Boyd and Fales (1983) offer a useful definition of reflection, suggesting that it is “the process of internally examining and exploring an issue of concern, triggered by an experience, which creates and clarifies meaning in terms of self, and which results in a changed conceptual perspective.” According to Siviter, 2004, p165) explains that reflection is about gaining self-confidence, identifying when to improve, learning from own mistakes and behaviours, looking at other people’s perspective, being self aware and improving the future by from the past. However, having knowledge about reflection does not necessarily mean that nurses are able to use reflection in a meaningful way during practice.     One day I was ask to transfer Mrs X from bed into wheelchair. Mrs X is paralised so to transfer her a full body hoist is needed. I had to call for help, it wouldn’t be safe to do this by myself. So while I was waiting for the other carer to arrive I have explained to Mrs X what and how we are going to do it. I have also cheked the hoist and battery if its fully charged and operational. I put right size sling on Mrs X with her cooperation. I carefully manovered hoist close to bed, then I hooked sling onto the hoist with short hooks on to of the body and long hooks on bottom. That was when my collegue arrived to help me with transfer.We asked Mrs X if she is ready when she said yes by pushing button on hoist remote I raised Mrs X up, while my collegue was holding her to stop swinging. When she was up I reversed away from bed. When I was in positionfor lowering Mrs X I have opened legs of the hoist. When my collegue manouvered wheelchair into position I was supporting Mrs X on back and knees. When wheelchair was in position my collegue took over of supporting Mrs X and I asked both Mrs X and my collegue if they are ready to lower Mrs X into a wheelchair. After positive answer I have lowered Mrs X into a Wheelchair and after that me and my collegue unhooked sling from the hoist and I safely moved the hoist away.     On my recent placement in Hairmyres hospital I had been asked by a nurse if I wanted to do the medicine round with them to get an idea of what it was like, I had agreed to this and found it interesting, during the round the nurse showing me was asked by another member of staff to do something, the nurse then asked me if I would finish putting two more tablets in the container and giving it to the patient, having read over the NMC code of conduct and remembering this statement from it “As a professional, you are personally accountable for actions and omissions in your practice and must always be able to justify your decisions”.     I started my shift at 7:15 am. My work clothes are comfortable and unrestrictive; my footwear is flat, sensible and offers protection to my feet and ankles. When I arrive to the service user’s home, I knock on the door and wait to be let in either by one of the service user’s or by a work colleague. Once inside I greet the service user, if they are awake. I then head to the sleep in room, to start verbal handover with my work colleague. After the 15 minute handover, I then check and sign the handover sheet. Check the on call rota, should there be an incident or or accident. Also check the communication book and also check any money and medication, then checking the activity planner. Once the service user wakes, we say hello to each other, and chat for a short period.
