On November 12, 1995, an American Airlines McDonnell Douglas MD-83 type passenger aircraft, which was operating as Flight 1572, departed from Chicago O’Hare International (ORD); however, the aircraft got substantially damaged due to impact to the top of oak trees on Peak Mountain Ridge in East Granby, Connecticut. The aircraft also hit the Instrument Landing System (ILS) localizer antenna, which was on its way to Runway 15 of Bradley International Airport (BDL). Some of these errors are of skill based, judgment and or perception based but some are a combination. 1. The flight crew initially boarded the wrong aircraft. A Comair ramp agent noticed that the accident flight crew had boarded the wrong airplane and started its auxiliary power unit. Another company ramp agent notified the flight crewmembers that they had boarded the wrong airplane. The flight crew then shut down the APU and proceeded to the correct airplane. The crash was ultimately caused by pilot error. A pilot that thought he could do anything and ignores everything he learned in his life just to show off. The pilot was Lt. Colonel Bud Holland, he was very reckless when he flew and everyone knew that he was, many people were scared to fly with him. Holland claimed that his signature high-pitch-angle climbs would pop rivets out the aging aluminum skin and by the end of his career he becomes even more reckless, he would push the aircraft beyond its limits. The Johnson and Johnson mission statement put customers first and stockholder (profit) last. Doing this forced the staff to work more hours because there was less staff in order to make a larger profit. These cutbacks to increase profit forced the crew to be overworked and exhausted. If they followed the mission statement from Johnson and Johnson the crew would have been much more alert. I also think that The Captain who sailed the ship out of the channel would have delayed the sailing at the first sign or smell of booze, which would have completely prevented the accident.…

When a major incident occurs in the aviation industry this has to be reported to the right person, authority, organisation and health and safety procedures must be followed. If there was a serious air crash this would be reported to the AAIB, CAA and the HSE. All of these organisations would document all factors of their investigation and put in place key factors and procedures to follow to ensure that it doesn’t happen again.…
