I. ACTIVITIES:
A. Cooperation with the Community Health Team on implementation of various projects/programs/systems and activities related to Pantawid Pamilyang Pilipino Program * Assisting the Rural Health Midwife on giving pre and postnatal care to pregnant women; vitamin A, deworming pills, immunization, and weight monitoring to children * Verifying/validating the Compliance Verification System (CVS Form completed by the Rural Health Midwife B. Participates with the Rural Health Midwife in giving vaccination to the targeted senior citizens * Assisting the Rural Health Midwife in actively masterlisting senior citizens eligible for vaccination in the community * Conducts interview with the targeted senior citizens before the inoculation for determining contraindications to influenza and pneumo vaccines C. Validates/verifies compliance of the Pantawid Pamilyang Pilipino Program beneficiaries to the conditionalities of Pantawid Pamilyang Pilipino Program * Providing written documentation of 4Ps member compliance using the Planong Pangkalusugan Form * Creating awareness/motivating the 4Ps beneficiaries to submit themselves for the next schedule of follow up check up D. Managing the new lists of households with National Household Targeting System ID * Mobilization of Community Health Teams * Dissemination of Planong Pangkalusugan Forms E. Participates in the Family Development Session * Cooperates with the Rural Health Nurse and Rural Health Midwife in discussing the objectives, benefits and different methods of Family Planning F. Managing the Planong Pangkalusugan Form of each targeted household - Tallying of significant data for weekly reports

The start of August gave me the core challenge I had so far in my career however had gladly worked on and survived before the month ends. My 7 current barangays should be given up to my
