Mobilize The first step is mobilize which correlates questions to ask and answer and include the mission and vision as well as potential partners and understanding the reason behind the question being asked (Healthy People 2020, 2017). This health assessment leads to pertinent questions regarding the use of screening tools and populations that need more focus. What is the colorectal and cervical cancer rates and what ethnic age groups do they most affect? Another pertinent question pertains to the amount of people that are screened for colorectal and cervical cancer and how this directly affects overall cancer rates. Initially, the provider should start with “mobilizing key individuals and organizations into a coalition” that can contribute to the overall goal (Healthy People 2020, 2017, para. 2). The overall goal of this health assessment is to assess key preventative screening methods in order to minimize the risk for colorectal and cervical cancers. Working together to set priorities and develop a realistic approach will provide the team with a sense of what they are able to do (Healthy People 2020, 2017). The data collected for this health assessment focuses mainly on preventative services related to colorectal and cervical cancer and how these statistics directly affect the number of people diagnosed with the disease in Ingham County. This collected data is considered the assess portion of MAP-IT and helps to track the progress of certain programs and interventions. By gathering a baseline of colorectal and cervical screening as well as incident rates, members of the team have established reliable data that can then be compared to the results after an intervention or program has been put into

    In 1990 Hartford Foundation funds, “Community Health Management Information Systems.” They gave grants to seven states and cities to develop those early prototype HIE’s. HIE focuses on quality assessments and cost reduction by streamlining patient eligibility information for billing. The problems of HIE are immature technology including slow internet connections and data integration. By the mid 1990’s Community Health Information Networks focused on cost savings associated with moving data between providers, decentralized architecture to address privacy concerns. Most CHIN’s failed at this. Between 1990-2001 the Institute of Medicine reports on patient safety and quality.
