Second Transition Face To Face Meeting
CM conducted a 2nd Transition Face to Face meeting for Brandon (youth) in front of youth’s school, St. Aloysius Elementary Academy in Jersey City, NJ. In attendance were Jasmine Alexander (CM), Brandon Whitmore (youth) and Ms. Lee (caregiver). Ms. Lee informed CM that youth has made improvements in the home and community. Youth reports he plays on the basketball team for St. Aloysius and he enjoys playing sports. CM was informed youth has basketball practice once a week on Mondays from 4pm to 6pm. Caregiver stated youth has a Big Brother mentor and youth continues to go to outpatient at JCMC. CM and family discussed medication; family reports continues to take his medication regularly. Caregiver informed CM that youth has been behaving in school
