Self Reflection On Hospital
Today was my third week in Metro Hospital. I was assigned one patient to work on time management with my clinical instructor. The patient I worked had surgery because she had a periocciptal aneurysm that had not yet burst. She chose to have the surgery because she learned the risk of having an aneurysm. I have learned that first thing I need to do is to check to see if the patient is alive because it is my responsibility to care for the patient. But I read the doctor notes prior to understand the priority assessment for the patient which is q4 neuro check and incision assessment. I thought I was able to prioritize my assessment well. As I was guided through the day, I was able to finish everything by 9:30. Now I could finish reading in the doctor’s note about my patient’s surgery. I thought this plan was very helpful because it was clear, which made it easier for me to plan my care for my patients. My weakness for this clinical day is confidence, although it is improving day by day. As I continue this program, my ability to critical think will improve as well as my confidence. The priority of care was to assess his neuro status. Talking to him, he seemed to have a colorful personality. He was talking to me about how he was a “player” when he was growing up. Now that he has grown up, things has slowed down. When the nurse was passing meds to him, she would ask the general questions that nurses would usually ask to a patient such as “would you rather take your meds with water or juice”, and the would respond “why don’t you use common sense”. I liked how she was able to tolerate the patient’s shenanigans. Just talking to the patient, I need that he was adamant in what he wants. I remembered helping my patient get to the bathroom after asking if he wanted to use the bedpan. He was so adamant about using the bedpan, so wanted to use the toilet because it would cause “less mess”. I helped my patient get out of bed to use the toilet. As weak as he is, I needed extra help because he was super heavy. I put him down for a minute, as the nurse aid came to my assistant to help my patient in the toilet. He was in severe pain when we ambulated him. His blood pressure elevated, which showed that he was in severe pain. For next time, I know now to encourage the use of bedpan regardless. Overall, I thought today was a busy and successful day. I was able to time manage properly and assist my patient for needs. I helped Alyssa clean her patient and got my careplan

    My first day with my preceptor this week (Wednesday October 26, 2011) was cut short due to a school obligation and personal situation. Although I only spent five hours on my unit, I did my best to make them count. I was particularly excited for my patient assignment today because I was caring for two infants which is truly my passion. My first patient was the same patient I had cared for the previous week, a now 21-day-old baby boy admitted with a positive urine culture and blood culture. I was glad to have this patient again because I was able to further build on my relationship with the patient’s Mother and reinforce the education I had provided previously. While caring for this patient I think I demonstrated the clinical competency of evaluating the effectiveness of the patient’s plan of care utilizing critical thinking and the nursing process and revising the patient’s plan of care. For example, this patient was responding well to his antibiotic treatment every six hours, as well as consistently taking in adequate amounts of formula with each feeding. While calculating the intake and output I noticed that the patient was urinating excessive amounts. After assessing the hydration status of the infant, I questioned the primary physician regarding the need for the client to be receiving maintenance fluid at a rate of 10ml/hr.
