The nursing profession requires an individual to be alert, watchful, and prepared. They must monitor patients closely. Concentration and attentiveness declines when a nurse is overloaded with work. Nurses who become fatigued from the work overload can become a danger to themselves and their patients. Medication errors and pressure ulcers are common results from fatigued nurses. Medication errors happen a lot when a nurse loses the ability to concentrate and focus. Pressure ulcers are a result of poor nursing care which can be caused by fatigue. A nurse may be so fatigued by the end of her shift that she does not properly position a patient. Basic care is sometimes put on the back burner, or is delegated to less qualified staff. As well as providing basic nursing care, a nurse must also give report, check patients orders, medications, and labs, all in one shift! One of the key nurse managers’ key competency is practicing the AACN essential I that uses written, verbal, nonverbal, and emerging technology methods to communicate effectively with a variety of disciplines to inform decision making by using inquiry, analysis, and information literacy to address practice issues ("Essentials," 2008, p. 12). This clinical objective was observed being applied in a clinical setting by my preceptor who is the Labor and Delivery manager in Valley Hospital. The clinical staff supervisor reported that one of their patients’ needs who has a complicated health issues were not taking care of which was already being known. The patient cannot have delivery in the L&D due to her complex health issue but the doctor doesn’t want to send her to the O.R. for surgery. The manager talked to the involved doctor and discussed the situation with the doctor and clinical staff supervisor (CSS). They exchanged suggestions and clarified the doctor’s reason for not agreeing to send the patient to another floor. After the discussion, the doctor agreed to send the patient for surgery in another floor. Nurses are also responsible to assess fatigue related symptoms that could put them and others at risk and intervene immediately (Phillip & Maffett, 2014). Risk management can assess for fatigue-related risks such as staff working overtime, being under staff and policies that encourages over time. These extra hours will play a toll on our current employees who are expected to step up and take these extra shifts. “Long work hours can lead to fatigue, restlessness, inadequate sleep, pain and deficits in performance and reaction time as a result of increased exposure to physical demands and insufficient recovery time (Bae, Nursing Overtime: Why, How Much, and Under What Working Conditions?). Continuing, “Reduced rest and recovery time leads to physiologic depletion or exhaustion that continues into the next workday. The profession of nursing requires a capacity and joy for caring and healing others both mentally and physically. Nurses spend their careers caring for patients and their families often in the worst and most frightening periods of their lives. Nursing responsibilities can be lengthy, stressful and physically and emotionally demanding. The demands of the nursing profession coupled with the nursing shortage and longer work hours put even more stress on nurses. Despite these extreme demands, many nurses do not fully appreciate the importance self-care. Yet without proper care for themselves, nurses are not able to provide the best care for their patients. “Self-care and self-healing is a critical component of being able to provide holistic nursing because holistic nurses recognize that they cannot facilitate healing unless they are in the process of healing themselves” (Cohen, 2010, p.1). For this reason and many more, nurses need to allocate time and energy for caring for their own needs as well as needs of their patients.
