Making the nursing staff aware of the neglect of postoperative pain management in the patient population would be a step in the right direction to making a change. In the future, the issue of turnover is and will continue to be a problem for the NICU and ACH as a whole. Many new graduates are joining the NICU in pursuance of experience, subsequently, returning to school to advance their degree or take a travel nursing assignment. This is a major cost to the ICUs, due to the amount of time orientation for a new nurse requires. Simultaneously, these nurses also receive their Neonatal Resuscitation Program and Pediatric Advanced Life Support certifications, another useless expense. Consequently, this turnover creates nurses being pulled from other ICUs, Intermediate units, and Resource Nurses into the NICU. Nurses can set numerous goals that counteract the detrimental effects of shiftwork that ultimately can improve their professional nursing practice. Nurses should bond with co-staff. Bonding with staff will help increase the likelihood of the shift running more smoothly. This increased cohesion at work may also protect employees against negative health outcomes (Treuer et al, 2014). Shiftwork shift sequencing can be broken down into three categories: clockwise rotation, counter clockwise rotation, and rapid rotation. As was stated, the nursing shortage will likely increase due to an aging workforce that will retire in the next five to ten years (King, 2014). Also, nursing schools do not have enough capacity to enroll all the qualified students that want to take up nursing as a professional. The United States Registered Nurse Workforce Report Card and Shortage Forecast researchers recently released a report in which they pointed out 12 states in the country that have a severe nursing shortage-Florida, Georgia, Texas, Virginia, Alaska, Arizona, California, Hawaii, Idaho, Montana, Nevada and New Mexico- each state was given an ‘F’ criteria (Oliver et al., 2012). In these states, it was found out that there were fewer nursing schools and this meant that there were fewer nurses. When staffing is inadequate patient safety, infection rates, patient satisfaction and staff satisfaction are only a few of the areas affected. Many hospital administrators and lawmakers want to make patient care a numbers game. The Joint Commission on Accreditation of Healthcare Organizations requires hospitals to create a staffing plan that defines staffing effectiveness as the skill mix, competence, and effectiveness related to the services needed. Besides the investigation between two nurses about shift exchange, I must find the holistic approach of the individual nurse. According to transformational style, a leader must be concerned about the individual needs of each nurse and to do this by working one-on-one in a private manner. It is important to understand their situation, needs, and also to figure out whether nurse had sound reason the shift change. I would communicate with the nurse in a calm manner to find out what was the struggle, in the nurse who could not come to work on her time of care. In addition, I would investigate the other nurse to ask why she could not switch the shift. Also, I think it is important to understand the whole picture, not just one side. Since this is fair treatment, the two nurses, they will be happy and willing work to help promote safety and patient care. Kerfoot (2015), agreed with this approach when he stated, “Happiness starts with an infrastructure that supports the employees to do the work they are inspired to do.” I believe transformational leaders need to be knowledgeable about diseases process and patient care.
