The Royal Pharmaceutical Society (2016) suggest that patients use medicines more than any other interventions to manage their medical conditions. When prescribed, and used effectively medicines have the potential to significantly improve the quality of lives and improve patient outcomes (Royal Pharmaceutical Society, 2016). Prescribers face many challenges in ensuring the correct medicine is prescribed and patients are supported in using them effectively. Evidence suggests that there needs to be an improvement in the way this is addressed (Royal Pharmaceutical Society, 2013, National Institute for Clinical Excellence, 2015).
