Social Work Case Study: Biweekly ILP Meetings
On November 25, 2015 Client Danielle Bush and her baby daughter Jazmine came to Social Services office for her Biweekly ILP meeting to be conducted.
Client's exit strategy is to seek and obtain housing by 3/2/16. Client must seek for employment and submit search log until she secure employment by 1/2/16. Client is expected to comply with savings and have saved enough for one month's rent and security by 2/2/16.
Housing: Client was reminded that she must meet with her assigned Housing Specialist, comply with all housing requirements and explore all available housing options.
ILP Meetings: Client missed ILP meeting since October 6, 2015. Client was reminded to attend all her weekly scheduled ILP meetings or client will be issued a written warning

Client is currently mandated by HRA to participate in BTW program with the dept. of Parks.
Medical: Client stated that her daughter do no longer has the skin rash the she has at our last meeting. Client was Client was made aware that she must maintain all health benefits in order to schedule annual physicals and seek medical attention when needed. Client is expected to submit copy of her physical by 12/7/15.
Childcare: Client needs childcare for her daughter Jazmine has been instructed to apply for childcare at HRA. Client must ensure child safety at all times.
Education: CM referred client to Bronx Educational Center for the GED program. Client is expected to enroll and submit proof of enrollment by 12/7/15.
ACS: Client receives preventive services and her Worker Keyla Richards she stated that client goals is to be enrolled in a job training or educational program as soon as possible
Room Inspection: Client room has been disorganized. Client is expected to pass weekly room inspections.
Fire Drills: Client is expected to attend all facility fire drills, failure to do so may result in a call being made to the State Central

Client was reminded that she is expected to return to the facility everyday by 9pm. Client will be issued a written warning every time she violates curfew.
Rules and Regulations: Case Manager constantly reminds client that her compliance with the rules and regulations is expected. Case Manager will constantly remind client that she is expected to comply with the client code of conduct form she signed upon arrival to this
