Strengths Challenges
Sarah has a brother that cares about her. It appears that she is not sticky to her regular regimen of antipsychotic medication.
When she is taking her antipsychotic medication, she appears to be stable. Communication/speech is remarkably getting worse.
When on a regular regimen of antipsychotic medication, her quality of life improves. Limited social support and remaining in social isolation. Her Brother (Jack) fears that his two children might be in danger when Sarah is near.
From analyzing schizophrenia, it appears that the prevalence rate is less than 1% of the general population. All races, sexes, and socioeconomics statuses can acquire this mental disorder. Typically, onset of this disorder is seen earlier in girls (twenties) than boys (mid/late twenties). Also, with this population, the clinician should consistently monitor a client’s depressive episodes. Suicide ideation and commitment to action are high. When the client is on medication, she appeared well in a care program, involvement with a local restaurant and involved in social engagements. Without medication, the client’s environment becomes a catastrophe. Client has even disappeared for days on end.
Goal 2: Increase client’s overall functioning in areas of social and/or occupational.
Objective 2a: Once medication regimen has been stabilized, the client will increase activities in social settings with aid from the ACT (Assertive Community Training) team.
Intervention 2a: Social worker will conduct motivational interviewing skills to get the client to change behaviors of social isolation.
Time-frame: Client will attend therapy sessions for an estimate of 8-10 sessions or until termination of mental health services.
Objective 2b: Client will increase activities that are positive and fulfilling.
Intervention 2b: Social worker and client will identify cognitive deficits that are hindering the client’s ability to thrive in areas mentioned in Goal 2.
Time-frame: Client will attend therapy sessions for an estimate of 8-10 sessions or until termination of mental health
