A. Brief description of client:

Suzie presents herself as a 45-year-old Caucasian unmarried female with short hair and was wearing male-typical professional attire and glasses. She came across apathetic, chewing bubble gum and seemed to have a very relaxed attitude. Suzie later identified that she had a long-standing partner, Samantha, who whom she resides and does not have any children. Suzie graduated from a community college and is currently employed with Steelcase, where she works on the line. She did not describe herself as religious or spiritual, however is trying to find a local church willing to accept her sexual orientation.
B. Chief complaint and symptoms by client and others present (at beginning and at latter time):

According to Suzie, her partner, Samantha has asked her to cut back on her drinking multiple times, which Suzie does not foresee being a problem. Suzie stated that she does not have a ‘drinking problem’. She claimed that she wanted her working conditions to improve, as she states she is viewed at as the ‘trouble maker’ at work. She is eager for a promotion up for dibs at Steelcase and hopes her coworkers can stop starting fights with her and keep their thoughts to themselves.
C. Context of the referral, precipitating events, facts, and dates:

Suzie initially reported that Steelcase referred her to get an assessment and seek help after she got in a quarrel with a coworker, and then had to drop. It was then that Suzie was caught and dropped dirty for alcohol and marijuana while at work. According to Suzie, the union agreed to let her get help and get clean instead of firing her. Suzie stated that her co-workers “talk shit” and call her a ‘dike’ behind her back

    After graduation high school Orman decided that she wanted to go to college to become brain surgeon. Her SAT scores were not high so she applied to the University of Illinois at Urbana-Champaign and much to her amazement she was accepted. Upon her arrival she met with her guidance counselor and explained her dream to become a brain surgeon.     “A state of complete physical, mental and social well-being and not merely the absence of disease” is the World Health Organization’s (WHO, 2012) well known definition of health. It is understood that health is referring to one’s physical state however it is also unnoticed that mental state is also a determinant of health. The general public has little or no knowledge about mental illness, as it is often a sensitive subject. Furthermore, mental illness is completely invisible unlike most general health conditions. Nurses have a responsibility to be advocates for patient health as a part of their practice. The Canadian Nurses Association (CNA) outlines a list of competencies which nurses must maintain and meet within their practice. The CNA framework (2010) specifically mentions supporting healthy public policy and social justice in nursing practice. This includes working to regulate and support those affected by mental illness stigma. As practicing student nurses, we understand that it is our role in the health care system and within our communities to raise awareness about mental illness stigma and promote prevention. As a result, we have chosen to research and present our information and knowledge on substance abuse and addiction as a part of mental illness to a grade nine class.…
