cigarettes and started an argument over his drug use. Mr. Waddell states, "When I went to my sister house her son-n-law started with me to and I'm on oxygen, how I suppose to defend myself with that boy half my age."He admits to threatening sister's son-n-law with knife. Mr. Waddell does not appear to be exhibiting signs of agitation, aggression, or responding to internal stimuli.

Waddell sister and son was contacted for collateral information. Mr. Waddell son reports for the pass 12 years his father has had substance abuse issues. He reports his father has been on "crack" cocaine and he is afraid his father might overdose on drugs and die. He reports a similar statement about the altercations tonight,however mentions he believes his father was about to buy some "crack" cocaine at the gas station and tried to run him over when he was attempting to leave. So, he proceeded to punch out his window in anger. He sister, Tammy reports an altercation occurred at her home tonight. She reports Mr. Waddell has been asking multiple family members today for $20 and they believe he was just trying to by use drug. She reports her son-n-law did slam her brother, then Mr. Waddell went into the kitchen to get a knife. She states," I have dull knives and no one was really hurt. She suspects her brother has been on drugs for 15 years. She reports since he loss his wife 2 years ago he substance abuse issues have worsen. She states, "We just want him to get some help, we are just overwhelmed and tired dealing with
this."
According to Mr. Waddell most recent UDS he tested positive for Benzodiazepines and cocaine. He reports a history of smoking marijuana since the age of 13, a joint a day, however stopped smoking marijuana 14 years ago ]. Mr. Waddell states he is prescribed Benzos and denies abusing them. He states," I just started snorting some, you know, 3 weeks ago, about a half a gram. Mr. Waddell reports last cocaine use was 3/18/17. He denies experiencing withdrawal symptoms.
This information was reviewed with Dr. Snyder and Andrew Holst, LCSW. They both shared the disposition that the patient should be discharged pending his agreeing to follow up with outpatient services. Patient has agreed to follow up with outpatient services as recommended. The patient informs this clinician he only wants his son made aware of the plans concerning him. TACT attempted to contact Waddell Jr. (336-954-6065), however no answer. A voicemail was left to call TACT back ASAP. Patient contracted for safety, was given outpatient referral information, and completed a crisis plan.
