Late Entry

On 2/11/2016, CM met with the client to complete Bi-Weekly ILP Review. Client was dressed in gray pants, pink shirt and black jacket with a multicolor knit hat. She was cooperative, friendly and happy. CM observed that the client face cheek was a little swollen. CM inquires the reason client face is swollen. Client reported is because she didn’t have medication for a few days.

SOCIAL UPDATE: Client reported she went to the pharmacy at Broadway to refill her medication and she could get it refill because it was too early to refill and she run out of meds. She also mentioned the onsite psychiatrist called the pharmacy and the pharmacy mentioned it was too early to refill client medication but the gave the client a few pills

MEDICAL UPDATE: Client continues suffers from uncontrolled seizure disorder and she reported no seizures this week. Client need to find a PCP, and GYN, Ms. T. Morgan/CAMBA Home Health Care Coordinator is assistant client to locate medical providers. She also will escort the client to each appointment.

SUBSTANCE ABUSE: CM inquires if the client abuses any substance this week. Client replies; No" and she is not interested in participating in a substance abuse treatment program.

MENTAL HEALTH UPDATE: Client reported she called NY Psychotherapy Mental Counseling to address her Major Depressive Disorder, Single Episode, in remission (Primary), PTSD (Post Traumatic Stress Disorder).

HOUSING UPDATE: Client is 2010E and housing package completed and circulating. She is also a candidate for SRO with 24 hour case management on site to provide the client with medical help and to monitor client medication. Client reports she will go see SRO, but she will not accept it.

REVIEW ILP AND CLIENT RIGHTS AND CLIENT CO.DE OF CONDUCT.
CM reiterates the shelter rule and regulation and client must adhere to the shelter rule and regulation and 10pm

    D-Client arrived on time for his scheduled appt. with this writer. The client was alert and engaging throughout the session. During the course of the one hour session, he was aware about completing his first initial tx plan, at which he was open to discuss and complete.
