Whether it is the failed response to Hurricane Katrina, or the issues with the rollout of the
Affordable Care Act website, public agencies continuously repeat the same mistakes, cementing their reputation as an inefficient delivery service. In his book The Next Government of the
United States: Why Our Institutions Fail Us and How to Fix Them, University of Pennsylvania political science professor Donald Kettl (2009) gives us two examples of public agency issues: 1)

The very first task is assigning accountability to a specific body. If, for example, nine private agencies are being contracted by the DHS to provide services to Area A, and some of these services might result in deaths of civilians or destruction of property, there must be clearly defined consequences for those in charge of these agencies. This can include the severing of the government contract, or any other such incentives for agency leaders to ensure successful execution of services. The agencies themselves must also have someone accountable for communication between groups, with sharing of information made thorough and routine. Although such a solution can be seen as infeasible due to various constraints in the public sector that were previously mentioned (such as employment termination issues), stricter punishments must be adopted and enforced
