CM spoke to Susan Buffield (EMYL Clinician) in regards to Tavian (youth). CM and Ms. Buffield introduced themselves. Ms. Buffield reported that youth is doing good at EMYL. CM and Ms. Buffield discussed transition for youth. Ms. Buffield reported that the upcoming treatment team meeting is on 11/16/16 at 11:00am and the team will discuss transition for youth. CM agreed.

    Prior to this, Yommala only had 11 left to complete. The concerns are that based on her caseload, many of her patient’s were missing treatment plan since 2015 and 2016. Senior Counselor questioned Yommala as to how this could have happened when in 2016, Counselors had three months to ensure treatment plans were current due to “ Smart” not populating the treatment plans.
