Identification of eligible studies
One hundred and ninety six and 970 studies from Medline and Scopus were identified, respectively; studies were duplicates, leaving 980 studies for review of titles and abstracts. Of these, 8 studies were review and extracted. Characteristics of the 8 studies are described in Table 1. Among 6 hook plate fixation studies, the comparator included TBW fixation in 3 studies, CC fixation (1 study of tight rope and 1 study in suture anchor) in 2 studies and plate screw fixation in one study. Comparing to CC fixation (suture anchor), the comparator is plate and screw fixation in one study. One TBW fixation study compared to TKW fixation. All studies were treated in unstable fracture distal clavicle Neer type II. Mean age and follow-up time after surgery varied from 34.3 to 50 years and 8.6 to 37.9 months respectively. Direct Meta-analysis
Data for direct comparisons of all treatments and outcomes measured at the end of each study are described in Table 1. Pooling according to outcomes was performed if there were at least two studies for each comparison, as clearly described below.
Treatment effects for CMS was moderately heterogeneity, in which hook plate fixation reached statistically significance with the pooled UMD of CMS of 4.16 (1.97, 6.36) higher when compared to TBW.
While CC fixation have higher UCLA score with pooled UMD of 3.20 (1.37, 5.03) when compared to plate and screw fixation. This suggested that use of hook plate fixation could be approximately 4 score higher functions when compared to TBW fixation and CC fixation could be approximately 3 score higher functions when compared to plate and screw fixation. In contrast, there were no significant treatment effects of CC fixation on CMS when compared to hook plate and plate screw fixation. Whereas UCLA score, there were no significant treatment effects of hook plate when compared to CC fixation and TBW fixation. The effect for complications was homogenous to mild heterogeneity, in which only hook plate fixation reached statistically significance with the pooled RR of 0.23 (0.08, 0.69) when compared to TBW fixation.
