Termination Process
I have begun the termination process with both my clients and supervisor. I have had several clients that I have worked with on the acute unit who were flagged as High Risk for Suicide that I had to terminate with prior to them being discharged. I have also carried a small caseload in which I started the termination process early (due to my supervisor having to attend a Suicide Prevention Conference in Denver this week). I would say the process was much easier than I expected. All of the veterans were extremely kind and thanked me for my service and wished me well in my future endeavors. I even had one who asked me to come back to work for the VA. That really made my heart smile.
As a result of my supervisor being at a conference this week,

The term constructive discharge is by definition when an employee feels they are forced to resign their job because the employer has made working conditions unbearable (Doyle,2013). In the circumstance presented, the employee felt compelled to resign because the work schedule was changed and would require him to work on his religious holy day. The business changed the production team schedule to accommodate growth and expansion, changing the schedule from 8:00am-5:00pm, Monday thru Friday, to four twelve hour shifts. The employees would rotate four days on and four…

A constructive discharge arises when a wage earner maintains the organization forced him/her to step down because working environments were made unbearable. The claim of constructive discharge under the Title VII Civil Rights Act of 1964 by Mr. X, is unjustified. The employee notifying the company that he was upset with the scheduling change in the production area never filed a complaint. Furthermore, Mr. X did not allow the organization the opportunity to respond to the claim. Thus, the organization was not aware the production schedule change created an unbearable working environment.…

Soldiers returning from duty who served in the Iraq and Afghanistan wars have the highest rate of suicide among veterans in our nation’s history. The rates post 2001, the real beginning of the USA’s involvement in these conflicts, have increased significantly more for the 2 branches of military that get more up close and personal with the conflicts (Army and Marines) versus those who fight from afar (Air-Force and Navy. In 2009 the US Army suicide rate was almost double that of regular civilians. While there are programs in place to help returning veterans, the Veterans Association (VA) is unorganized, outdated, and doesn’t do nearly enough to protect the mental health of those veterans. We need a new,…

Why is it difficult to perform a truly independent, objective review? In most cases those performing the review have some previous knowledge of the project, which presents opportunities for bias. Sometimes the review team, or facilitator, is perceived as a jury, but even jury members come with built-in biases. For example, internal politics have been known to enter into decisions concerning closure of a project. The simple point is that every attempt should be made to keep the review independent and objective. If the review of projects is a regular procedure for all projects, the negative stigma of audits is minimized. 3. Why should you separate performance…

The quality committee has worked closely with the hospital’s team of physicians and other providers as well as all nursing departments, ancillary department’s and all relevant nonclinical staff to propose the funding of a department, staffed by experienced registered nurses, who will be the key players responsible of bridging the gap between hospital and community providing discharge instructions, education and will follow the patient in the weeks coming after they leave the hospital ensuring access to medications and appropriate follow ups. The purpose of this program is to significantly reduce the number of readmissions that are related to ineffective discharge planning and/or poor education. The transition team will consult on every admitted patient in the hospital to assess their ability to effectively transition into the community setting after discharge. Those who have adequate resources, established relationships with primary care, and do not require any social work assistance, the team will still provide a comprehensive discharge planning evaluation, education, and an evaluation of their understanding of the steps to take as soon as they leave our building. Each patient will be given a score based on his or her evaluation that will be associated…

The first step to physician assisted suicide is to make a formal oral request and attend advocacy groups to make sure it is the right decision for the patient. Then 15 days later they have to make another formal request. Once they have made your first two formal requests, they have to fill out a written request form signed by two adult witnesses that are psychologically capable. If after they have made all the requests they need and get approved, then they move on to getting the correct prescriptions they need. Their physician will prescribe the lethal drugs two days after their final request and if they get approved. Their physician cannot administer the drugs them self because if they administer the drug it is illegal and they could face criminal charges. Most of physician assisted suicide take place inside the patient’s home because it is more comfortable and since their physician cannot give them the medication them self.…

One thing that stood out to me about the discharge process is that I asked what if the client is not in a suitable environment, he explained that the social worker can only suggest a better environment. If the clients deny then there is nothing that the social worker can do but respect the client’s decision. Proceeding with my next question I asked What are the most critical problems facing the agency’s targeted population. Mr. carpenter explained that the problems that they are facing were people living their facility and living at home alone. I asked him why was that a problem and he explain that things happen when they are home alone. Mr. Carpenter began to explain that older people tend to injury their selves such as falling, and if no one comes to check on them they could eventually die in that same spot. Another problem the agency faces are insurance company making it difficult for patients to get their medications. Without the medication that is needed could lead to serious life threaten…
