What is the priority nursing diagnosis for Marie, who is experiencing residual effects of epidural anesthesia?…
If the epidural anesthesia needed to reduce the pain of labor and delivery had been packaged in a bag whose cap did not fit onto the main IV, no one could have put it directly into the bloodstream instead of the epidural space around the spine. If, as in some hospitals, two nurses check and double-check IV bags before they are hooked up to the main pump, this reduces the likelihood of error.…
Internal fetal monitoring is accomplished with a fetal scalp electrode that is a direct electrocardiogram of the FHR and therefore produces the most accurate FHR tracing having an advantage over the external monitoring. The FSE is attached to the fetus during a vaginal exam and then connected to a fetal monitor. Because the risk of transmission to the fetus is increased by the small puncture in the fetal scalp, use of internal scalp electrodes should be avoided if at all possible in the presence of known maternal infections such as HIV, hepatitis or GBS. Fetal scalp monitors are also avoided in preterm infants because of the increased risk of ventricular hemorrhage.…
A number of barriers that affected implementation of comforting strategies for pain management during IV line insertion procedures in children were identified by the participating health care providers. The majority of the participating health care providers identified lack of time for implementation the strategies (42%), having an emergency situation and encounter (18%), and irritability of children (13%) as barriers that affected their ability to implement comforting strategies during IV line insertion. Additionally, health care providers identified; not knowing the right method to use (11%) as a barrier to implementing comforting strategies for pain management (Figure 1). Other barriers identified included; age, work overload, not expecting children to get much pain, lack of distractive materials at the facility as well as not thinking that it was important to control pain in this vulnerable…

The use of epidurals is so common today that many perinatal professionals are calling the 1990s the age of the epidural epidemic. Believed by many in the medical profession to be safe and effective, the epidural seems now to be regarded as a veritable panacea for dealing with the pain of childbirth. It is true that most women experience pain during the course of labor. This pain can be intense and very real, even for those who have prepared for it. But pain is only one of many possible sensations and experiences that characterize the experience of giving birth. Barbara Katz Rothman, a sociologist who studies birth in America, writes that in the medical management of childbirth, the experience of the mother is viewed by physicians as pain: pain experienced and pain to be avoided.1 Having experienced childbirth ourselves, we have great compassion for women in painful labors. However, we also feel a responsibility to mothers and their babies to explore issues concerning the use of epidural anesthesia in labor issues that are seldom discussed prenatally.…
The state of hypnosis may be described a state of deep physical relaxation, but with a focused mind. In this alpha brain wave state the unconscious mind can be more readily accessed, as some of the critical faculties of the conscious mind are temporarily suspended (Mantle 2000). The hypnotic state is known to be a phenomenon that occurs naturally, and we all enter hypnotic-‐like states for varying reasons, perhaps several times a day (James 2010).…
False labor is characterized by contractions that are irregular and weak, often slowing down with walking or a position change. True labor contractions begin in the back and radiate around toward the front of the abdomen. They are regular and become stronger over time; the woman may find it extremely difficult if not impossible to have a conversation during a contraction.…

In my opinion, the article was very helpful in the sense that it shows how physicians actually think of their nurses. They like that they are older and have a lot of experience. It gave me a good outlook on how they expect the nurses to act and hold themselves. It also gave me a little insight about how important it is to have good communication with your doctor. It will prevent your patients from any harm and it will keep you and your doctor a…
