Evaluation Forms
(Note: Students must bring a printed copy of the evaluation forms supplied in the syllabus and/or located on the electronic course forum the same day that presentations are due or the assignment will be subjected to the penalties outlined in the syllabus.)
Introduction of Self Speech/Get Acquainted Exercise by:_________________________________________
(Note: Completion of this speech carries no grade points in the course, but is required prior to further speaking endeavors. No repeats will be allowed.)
P=Pass, F=Fail
Objectives for students:
P F 1. Student gives a brief introduction-of-self speech by providing the audience with some insights into his or her background, personality, aspirations, etc.
P F 2. Speaks extemporaneously (i.e., using good eye contact, not tied to notes, and not memorized), using notes on the bottom half of this sheet of paper.
P F 3. Properly finishes the speech within 1 (minimum)- 2 (maximum) minutes.

A OK X 3. Speaks extemporaneously (i.e., using good eye contact, not tied to notes, not memorized, using one 4” x 6” notecard).
A OK X 4. Delivers the speech appropriately and well (i.e., good language, good nonverbals, appropriate enthusiasm, avoidance of verbal fillers or nonfluencies, etc.).
