Instructions:
1. The PharmCAS ID, MUST be provided on each item of the supplemental application materials.
2. The Supplemental Application Fee is to be paid by personal check or money order, made payable to UH
College of Pharmacy. Write your full name and PharmCAS ID on the memo line. Supplemental
Applications will NOT be processed and an application will not be considered for an interview until the $100.00 Supplemental Application Fee has been received. (Fee subject to change without notice.) 3. Any pre-pharmacy prerequisite credit earned by examination (if applicable) must be mailed to the UH
College of Pharmacy Office of Admissions with all supplemental application materials in order to be verified. Acceptable forms of credit by examination documentation are listed below: o Official, sealed AP/CLEP/IB Student Score Reports from College Board
(www.collegeboard.com)
OR o An official transcript in a sealed envelope from your college/university indicating earned college credit (i.e. AP scores, CLEP exams, etc.) at the institution may also be used. Specific course names and course numbers awarded must be clearly indicated on the transcript for credit to be recognized.
