TENDER FOR THE SUPPLY AND INSTALLATION OF RADIOGRAPHIC/FLUOROSCOPIC SYSTEMS, GENERAL-PURPOSE FOR NORTH DISTRICT HOSPITAL AND TUEN MUN HOSPITAL
LODGING OF TENDER
To be acceptable as a tender, this form, properly completed in quadruplicate and enclosed in two separately sealed plain envelopes, one marked as “Technical Proposal” which should include all tender submission except the Part IV - Contract, Schedule 25 (Contract Deposit) and Schedule 26 (Price summary). The other envelope marked as “Price Proposal” which should include the Part IV - Contract, Schedule 25 (Contract Deposit) and Schedule 26 (Price summary) only. Both envelopes should clearly state the subject of this tender “Tender for the Supply & Installation of Radiographic/Fluoroscopic Systems, General Purpose for North District Hospital (NDH) and Tuen Mun Hospital (TMH)” and addressed to the Chairman, Tender Opening Committee, and must be deposited in the Tender Box situated at Procurement & Material Management Office, Room D23, 2/F., Block B, Staff Quarter, 30-32 Ngan Shing Street, Shatin, New Territories. before 2:30 p.m. (time) on 28 May 2012. Late tenders will not be accepted. The Procurement & Material Management Office has implemented the five-day week scheme after 1 April 2007. Please do not plan to submit your Tender or sample on a Saturday or public holidays.

Tender Closing Time in case of Typhoon/Rainstorm In case a black rainstorm signal or tropical cyclone warning signal No. 8 or above issued by the Hong Kong Observatory is still in force or expected to be issued three hours before the Tender closing time, the Tender closing time will be extended
