Venipuncture: Blood and Vein
When blood is removed from a vein, it is called venipuncture or phlebotomy. Venipuncture should be performed with care. The veins of a patient are the main source of specimens for testing, the entry point of medications, and the site for IVs and blood transfusions. Because there are only a limited number of easily accessible veins in a patient, it is important that everything be done to preserve their good condition and availability. Universal precautions should be observed at all times. Gloves should always be worn when obtaining blood specimens. BEFORE BEGINNING ANY PROCEDURE—ACCURATELY IDENTIFY THE PATIENT ! Follow the proper protocol to identify the patient make positive identification of the patient by checking hospital armband for patient’s name and hospital number. Make sure the name and number correspond with the request label. When possible the patient’s identity should be verified by asking the patient to identify him or herself. Any discrepancies must be clarified before the puncture is performed. If the patient is not wearing an armband, tell the nurse you are not permitted to draw the sample until the patient is wearing an armband. Check to make the timing and other special conditions indicated for the tests ordered have been met. Personal protective equipment (gloves) should be worn during venipuncture. Be sure to use any additional protection indicated when entering an isolation room. Always wash your hands or use a hand disinfectant between patients. Make sure all the necessary equipment is on phlebotomy tray before entering the patient’s room. Item to check include: Alcohol pads, Dry and clean cotton balls, Tourniquet, Vacutainer, Collection tubes (make sure you have correct tube for all tests ordered and confirm that the tubes are not expired.) , Syringe of size sufficient to draw volume of blood needed, Safety transfer devices for use with syringe draws, Bandage tape, gauze, and/or bandages, Needles—The choice of needle will depend on the type of

    With the patient in the supine position after adequate prepping and draping of the left supraclavicular infraclavicular areas at 18-gauge needle was inserted in the left subclavian vein. A guide wire was passed through the needle and directed into the right atrium under fluoroscopy. The needle was removed and the incision was made in encompassing the puncture site. The dilator and introducer were passed over the wire. The wire and the dilator were removed and the catheter was threaded through the introducer into the upper portion of the right atrium. Using tunneler than a tunnel was made to a chosen exit site. We had placed a red dot in the general vicinity of the exit site. The catheter was threaded on the tunneler and pulled through the subcutaneous tunnel and out the exit site. The syringe adapters were placed on each tubing and secured with the locking sleeve. Blood could be aspirated and instill through each one easily. Each channel was flushed with heparin solution 100 units per cubic centimeter. A butterfly sleeve was placed on the catheter just distal to the exit site and it was secured to the skin with 2-0 silk sutures. The catheter was secured to the sleeve with a 2-0 silk tie. The course of the catheter was under fluoroscopy showed no evidence of caking. The look also appeared expanded the infraclavicular incision was clothed with interrupted 0 silk suture. Addressing was applied. The patient tolerated the procedure well and was sent to the recovery room in stable condition.…
