Your account has come to this department’s attention as a result of recent interactions that occurred at our Verizon Wireless retail location at 3825 Grand Avenue, Chino, CA 91710. Specifically, I have been advised that your account manager, Sarah Mitchell, entered our store on December 3, 2016, to get assistance with her phone. During the interaction with the store representative, Sara Mitchell became hostile and stated "If my phones end up not working tomorrow I will come back with a gun and shoot all of you" This behavior is disruptive to our business and adversely affects our employees and our ability to conduct business and it will not be tolerated. This will allow you time to port your number(s) to another carrier should you choose to do so. A final statement, which will be due and payable immediately, will be submitted to you within thirty (30) days. Until your service is terminated, however, any communications you wish to make concerning your cellular service and/or your account must be directed to our Executive Relations Department in writing via facsimile to 480-783-5083 or in writing to Verizon Wireless, P.O. Box 3190, Chandler, AZ 8544.

Except for written communication with Verizon Wireless’ Executive Relations Department, you are hereby prohibited from entering any Verizon Wireless location and from making further personal contact with any other Verizon Wireless representative regarding your cellular service and/or your account. Failure to follow this directive will be considered harassment and may result in immediate termination and additional legal action.

You are also prohibited from activating and/or maintaining service with Verizon Wireless in the future. Should service be activated by you in the future, Verizon Wireless reserves the right to terminate any such account
