It is evident that medication errors are very common in the health care setting. It can cause complications to the patient that may be detrimental and life-threatening to his/her health. In fact, this illustrates incompetence and an inadequate amount of focus, which are imperative to have in all aspects of health care involved. Unfortunately, there are various reasons as to why these problems occur. The responsibilities of the health care team and unclear knowledge of the information can influence this dilemma. To improve medication safety, becoming more cautious and efficient in work ethic can reduce medication errors.
