Analysis of Windshield SurveyAnalyze and summarize your observations and the input from key informants. Write a 2 paragraph summary of community health and safety issues for this community and strengthens of the community. Based on my observations of my community and input from professional individuals I have interviewed, concerns such as community health and safety are major issues in the Saint Louis, Missouri down town area. One of the biggest concerns is the increase of homelessness in the down town area. Per conversation with fire fighter Dyrle Wilson who is part of fire house 8 and a paramedic for the fire department states that with the increase of homeless individuals on the streets brings a boat load of health and safety concerns. When asked for Mr. Wilson to explain, he stated That the health issues were related to most of the homeless population, which he has been called to provide medical aid to. He also stated that most are dependent on drugs, most common alcohol. For the most part these homeless individuals have been living on the streets for years with not much food, clothing or medical attention. This not only is a growing concern for the individuals health, but safety for the persons providing aid to such persons. Without a clear understanding of the individual health issues or drug dependency it may be harmful to render aid to such persons without the fear of knowing if the individual uses drugs by way of injections, creating a fear of contracting aids, or if the individual will be combative due to the use of alcohol. As the homelessness continues to grow it has also caused a growing concern for business owners in the area. Some of the main concerns are loitering, pan handling and theft. Most business owners believe that their customers and business image are most affective by these people. Although the homelessness issue is a concern, the city of Saint Louis has begun to provide resources to aid with food, clothing, bathing, free healthcare and housing. One such resource center is the Saint Patrick Center which helps with all of the above. However the primary function, first and foremost for Saint Patrick is to find housing for the homeless. Through free educational programs most homeless individuals are able to secure jobs thus making it possible to obtain healthcare as well. Also by using the train the trainer method once homeless individuals are able to reach out and help other homeless persons to secure the same means in which they have achieved themselves. This is due to the fact that they once were in the shoes of the very person they are now helping. With everyone from city to government officials to communities helping to raise funds to keep such programs going shows the strength of the community and nation. Grade 88. For current, quarterly unemployment data, visit the Missouri Department of Employment of Economic Development. -Employment and lack of employment influence a variety of social and health risks, including access to health care insurance and physical and psychological needs. For example, individuals in office-based occupations are at risk for repetitive stress injuries and musculoskeletal disorders due to the sedentary nature of this work -Individuals who are unemployed or experience job insecurity may face health risks such as increased blood pressure and stress. Racial/Ethnic Characteristics Remember compare objective data for your community with local (city), state, and national data. -Gender distribution The Asian at 2.9, Black 49.2, Whites 43.9, Hispanic/Latino 3.5, American Indians/Alaska Native 0.3 and White only not Hispanic or Latino 42.2. These percentage are solely based off the 2010 Census report for Saint Louis County Population decreased at a rate of , -(0.3) for county and 0.9 for the state of Missouri year 2013. Economy poverty rate, unemployment rates, median income. Remember compare objective data for your community with local (city), state, and national data. -Income level influences an individuals access to health insurance and health care. Rates of uninsured can be difficult to measure. Wide variability across racial and ethnic groups exists. Historically, white populations have been the most likely to be insured and Hispanic/Latino populations have been the least likely to be insured. -Income level is also used to determine poverty status, which can determine eligibility for various assistance programs. Poverty status is based on a minimum level of income necessary to achieve an adequate standard of living. Poverty is on the rise in Missouri. According to federal poverty guidelines, the poverty threshold in Missouri in 2012 equaled 23,050 for a family of four. Families whose income falls near or below this amount may be eligible for medical assistance and other social service programs. -Median Income per capita money income in the past 12 months (2012 Dollars) 22,551 county, 25,546 state -Person below poverty level, percent, 2008-2012, 27.0 county, 15.0 state. Education High school completion rates higher education statistics. Remember compare objective data for your community with local (city), state, and national data. -The number of high school graduates in Missouri is projected to decline from 65,073 in 2010 to 59,727 by 2017. From 2017 to 2023 the number of graduates is expected to increase slightly, but will remain below the 2010 number of graduates. As Missouris population continues to become more diverse, students of color will comprise a larger share of high school graduates in the future. The percentage of nonwhite graduates is projected to grow from 16 in 2010 to 23 of all graduates in 2023. For all races, St. Louis County data indicate a higher percentage of individuals receiving at least a high school diploma, 30.5 compared with state level rates of 27.8. Attainment of a Bachelors degree in St. Louis County, at a rate of 17.6, was lower than state level rates of 21.3. All above information was resourced from the following web site listed below HYPERLINK http//www.culturecareconnection.org/documents/StLouisCounty http//www.culturecareconnection.org/documents/StLouisCounty Identify two Morbidity and Mortality/Risk Problems (Based on your observations from Week 1 subjective assignment, windshield survey and key informants provide Morbidity, Mortality (MM) and risk data for your community Example 1 if you observed obesity in your community then present obesity rates and health conditions associated with obesity i.e. CVD, HD, Depression, etc.). Example 2 smoking/drinking rates and health conditions associated with smoking i.e. Lung Cancer, COPD, etc. These variables may include the following gender (who does this problem affect the most, men or women) age distribution ethnicity or racial group(s) mostly-affected morbidity and mortality rates any data related to life-expectancy in light of the health issues is there a geographic area where the health problems mainly exist is there a relationship between the health issue(s) and environmental health hazards are these life-threatening health issues Remember compare objective data for your community with local (city), state, and national data. Homelessness Morbidity -Health status data concerning Homelessness and factors contributing to the incidence of disease drug abuse and depression revealed the following Homelessness Morbidity A need exists for increased efforts to provide health care, housing and resource centers in the general population, as well as an awareness of homelessness trends in populations of color. -Another Morbidity of concern is the vast majority of smokers in the city limits. -Behaviorial risk factors, such as use of alcohol and tobacco, diet, exercise, and preventive health practices play an important role in determining a persons overall health status. Control over such factors can reduce a persons risk for illness, disease, and premature death According to the latest available data from the Missouri Department of Health (2009), St. Louis County residents are at higher risk for behavioral factor such as hypertension than any other areas in general. In Missouri, the top three behavioral risk factors are obesity, hypertension and binge drinking. The impressions were taken from the most current 20122013 census report. What cause-and-effect relationships did you discover/identify As stated before, all cause and effect were solely due to the high poverty in the state relating to poor healthcare. Problem Analysis for Community/Zip Code Problem List two problems within your community (i.e. Infant Malnutrition, CVD, Lung Cancer, Obesity, Alcoholism, Violence) NOTE These problems need to be something the Community Health Nurse can impact (i.e. A CHN cannot build a sidewalk, repair a street, implement a new bus route, etc.) Homelessness_______________________________ Smoking____________________________________ Select one of these problems and complete the Problem Analysis chart Problem CorrelatesRelationship of Correlate to ProblemData Supportive of Relationship Smoking/second hand smoke/Lung CancerLarge number of restaurants and areas downtown that are not smoke-free as well as several tobacco bars/smoke shopAlmost every restaurant downtown has smoking available which cause health risk to those who smoke and those who do not have to dal with second hand smoke. Downtown community stay crowded and someone is alwaysThese are factors that contribute to problem and outcomes. Refer to appropriate sections of the database and relevant research of the findings in current literature. Data in the third column could be local, state, or national statistics. Refer to text (pg. 420-423) for additional examples. Community Health Nursing Diagnoses Based on your subjective and objective assessment findings above for both Weeks 1 and 2, write TWO priority community health nursing diagnoses. You must use this format as outlined in your text page 413 (do NOT use NANDA) Health Risk/problem of (specific injury, illness, or potential of, complication of, etc) among (specific population) related to (supporting evidence from your assessment findings). Your CHN Dx MUST be something that a community/public health nurse can do something about. See example below. Note Poverty is NOT a CHN dx Remember do NOT use NANDA Nursing Dx For example Risk of asthma complications among children in SE Dallas County related to observations of overcrowded living conditions, lack of public transportation (may result in inability to access health care), high number of animals observed living in homes, poverty indications (may result in lack of access to medications).
Community Health Nursing Diagnoses 1 Risk of spread of infections disease like Hepatitis, among the homeless community, related to the large homeless people population being drug users and sharing needles and not having the ability to obtain clean needles. Saint Louis has a large population of people with some form of hepatitis. Community Health Nursing Diagnoses 2 Risk for malnutrition, among the homeless community, related to the large population of homeless population seen getting food out of the local restaurants trash cans, as well as digging out of the trash cans on almost every corner. There are several homeless shelters, however the overwhelming homeless population in my community makes it difficult for the shelters to adequately feed everyone.Reference Module 3 (Week 3) Part Three Community Health Nursing Intervention Directions Please complete the following information on this template.
