Youth Unlimited Group Home Case Study

Mr. Farmer is a 12 year old male who presented to the ED via LEO following behavioral issues at Youth Unlimited Group Home and stating he was going to drown himself to staff. At the time of the assessment Mr. Farmer is calm and cooperative. He denies suicidal ideation, homicidal ideation, and symptoms of psychosis. He states, "I got mad and started destroying my property, then they tried to restrain me, after they let me go I told them I was going to drown myself. He states, "I was not serious about harming myself, but I was mad that we couldn't go nowhere because somebody was here. Mr. Farmer reports a history of depression, however denies current symptoms. He reports no history of self harm, but a history of hospitalization for behavioral issues. Mr. Farmer does not appear to be exhibiting signs of agitation, aggression, or responding to internal stimuli.
Matt Faulkner, the director of Unlimited Group Home was contacted from collateral information.
He reports Mr. Farmer has been at their facility since January of this year. He acknowledges Mr. Farmer has been having behavioral issues at school and has been cussing anytime he do not get his way he run around the house. He informs this clinician Brandon from the group home was on staff during the time of the incident. Brandon called This clinician to inform me of the
incident.

This information was reviewed with Dr. Osborne and Howard McQuirter, LCSW. They both share the disposition that the patient should be discharged back to Youth Unlimited Group Home and continue with their services. The patient's group Home staff has been made aware of the plans concerning the patient. Patient contracted for safety, was given outpatient referral information, and completed a crisis plan.
