    Saturday morning, July 23, 1983, Captain Weir makes the flight to Montreal, Canada with no malfunctions. Once landed Captain Weir and Captain Pearson do an aircraft turnover with each other. Captain Pearson is now in charge and has the idea the aircraft has to be drip checked due to the fuel gauges being inoperative. While sitting in Montreal, a certified avionics technician category 38, Mr. Ouellet boards the flight deck to perform routine checks. As well as doing routine checks Mr. Ouellet verifies the previous certified aircraft technicians log book entry. He however becomes confused and decides to perform a built in test equipment check on the fuel processor. Before he does the test he resets the pulled breaker causing the fuel gauges to go blank. Not liking the results Mr. Ouellet removes the processor to order a new part. Upon discovering there is none available he returns to flight deck. Once here he is distracted by fullers and aircrew boarding thus forgetting to pull circuit breaker number…

    On July 19, 1989, a McDonnell Douglas DC-10-10 operated by United Airlines experienced a catastrophic failure of the number 2 tail-mounted engine during cruise flight. The catastrophic failure led to engine debris severing all three flight control hydraulic lines resulting in total hydraulic loss. Le syndicat des pilotes demande à son personnel de ne plus naviguer sur les Airbus dont au moins deux Pitots n’ont pas été modifiées. A la suite de plusieurs incidents les mois précédents, la société Thalès avait déjà mis en garde les utilisateurs. L’Airbus selon le syndicat devient instable lorsque les sondes cessent de fonctionner. Ce bug, toléré par Air France avant l’accident, aura coûté la vie à deux cent vingt huit personnes. Il avait déjà été reporté à plusieurs reprises bien avant 2008. Notamment dès 1998, les responsables de Lufthansa avaient rapporté un incident semblable lors du survol du Mont Cameroun, lorsque les températures chutent parfois au dessous de -70°. Prévues pour des températures de -50° les sondes Pitot, selon divers professionnels dont le syndicat des pilotes, ne respectaient pas les spécifications du cahier des charges. Il s’agit ici de négligence, d’avidité, de déni et de persistance dans le déni. Les conséquences vont au-delà et à contre-courant du but poursuivi, puisque à terme, c’est le marché des Airbus qui est remis en cause.…
