Significant Health Care Event In the paper the topic to be discussed is a significant healthcare event. There are many events that have an impact on healthcare. The significant event that will be discussed in more detail is managed care. The main points to be discussed in further details are how managed care relates to the changes in health care, has managed care impacted the historical evolution of health care, and based on beliefs and values do I agree with managed care’s significance. To fully understand the significant health care event managed care first needs to be defined. According to Shi, L., & Singh, D. (2012), “Managed care is a system of health care delivery that seeks to achieve efficiencies by integrating the four functions of health care delivery discussed earlier, employs mechanisms to control (managed) utilization of medical services, and determines the price at which the services are purchased and, consequently, how much the provides get paid” (P. 8).
Managed Care Relating to the Changes in Health Care
Health care costs are rising significantly throughout the years. In the 1990s rising cost in managed care grew rapidly resulting in consumers to keep up with these rising costs. Individuals who are paying health care insurance out of pocket are struggling to pay these costs. Conklin, T. stated, “Managed care-on the other hand-reflects a significant change in doing business that gradually reached most areas of the health care arena” (2002). With the changes in health care costs hospital admissions are reducing, increase of ambulatory care, out-patient surgeries, and home care (Conklin, T, 2012). Individuals’ deductibles and co-pays are increasing in result of managed care. There are many struggles that health providers face when working in managed care. Raiz, I. A. The results of managed care and the continuing evolution of the American health care system are both quantitative and qualitative. They range from a reduction in hospital admissions and stays to an increase in ambulatory care, out-patient surgeries, and home care from an emphasis on prevention and better decisions by consumers about health-related behaviors to the sometimes limited choices by consumers in selecting practitioners and in utilizing benefits from increasing limitations in coverage with higher deductibles and co-pays to the reality of a still significant portion of the population among the disenfranchised or uninsured and from quality of care and treatment to issues and concerns around trust. Four of these areas will be addressed in greater detail below. The Institute for the Future in Health and Health Care 2010 described three tiers of coverage in todays evolving health care system and projected how individuals and families may experience this changing system based on which tier of health coverage describes their particular situation (2000). Their observations are summarized below Tier 1 The securely enfranchised. The first group represents 38 percent of the population. It consists of empowered consumers with considerable discretionary income, who are well educated and use technology, including the Internet,, to get information about their health.     Managed health care is a system of health care delivery managed by a company aiming mainly at quality/value cost effective services provided to patients. It has been introduced with an intention to avoid paying for unessential facilities and services directly to physicians. It helps in forming an intermediate between patients and physicians in such a way that health insurance organizations pay the physicians from the premiums paid by patients to insurers for the services provided. This helps in monitoring how cost effectively the services are utilized. It is not to be mistaken as health care delivery at discounted prices. It’s goal is to provide quality care at affordable prices by restricting patient’s choice of physicians and physicians limiting their fees. People who seek care in America are mostly enrolled in health care plans such as health maintenance organizations(HMOs) and preferred provider organizations(PPOs).The less common plan people are enrolled in is point-of-service(POS) which has features of both HMO and PPO.HMO provides integrated and preventive care services to voluntarily enrolled families with low premiums within the network of doctors and hospitals that belong to the organization. It requires to select a primary care physician(PCP) who serves as a personal doctor to provide all basic health care services. PCPs include family physicians and internal medicine physicians. Some HMOs consider pediatricians(for children), gynecologists(for women) as PCPs. Before seeing a specialist or for a diagnostic service, it is required to obtain a referral from PCP(“gatekeeper”). If an outside specialist is chosen or referral is not obtained before seeing a specialist, no coverage for services is rendered by HMO.
