STANDARD OPERATING PROCEDURE for OPTICAL SERVICES

1. PURPOSE: To establish policies and procedures for patients to obtain prescription eyewear.
2. SCOPE: This SOP is applicable to all personnel assigned or attached to the Optometry section and to all eligible patients seeking optical services through the Optometry section.

4. PROCEDURES: Provision of prescription eyewear is an integral part of the optometric services provided by this facility. Individual patients will obtain their eyewear based upon the following factors.
a. Categories:
(1) Active duty military will have their spectacle prescription filled in standard military issue frames and lenses appropriate to their MOS and duties. One pair of “civilian-style” frames may be issued annually as part of the Army’s Frame of Choice Program.
(2) Retired military personnel may have their spectacle prescription filled in standard issue frames and lenses. (Unfortunately, at this time, the Army does not provide FOC for retired personnel)
(3) Other DOD sponsored individuals who require military issue eyewear will have eligibility verified by the Patient Administration Division prior to submission of DD Form 771, Eyewear Prescription to the servicing optical fabrication laboratory for filling at government expense. All spectacle orders originating from the Optometry clinic will be returned to the clinic before dispensing to maintain accountability.
