2. Asymptomatic patients with severe AS (Class C2) with Ejection Fraction< 50% with decreased systolic opening of a calcified aortic valve and aortic velocity 4.0 meter per second or greater or mean pressure gradient 40 mm Hg or higher (Level of Evidence B)
3. Asymptomatic patients with severe AS (Class C or D) when undergoing cardiac surgery or other indications and aortic velocity 4.0 meter per second or greater or mean pressure gradient 40 mm Hg or higher.
