Per medical report dated 08/18/2016, patient was prescribed with Norco.
Based on the progress report dated 09/06/16, the patient presents for ongoing evaluation of her chronic low back pain. The patient states that her back pain radiates down to the lift hip, and her pain is 10/10 when most bothersome without medication. With medication, her pain is able to be rated to a 5/10. When she takes her medication, she is able to get up and take a shower, she is able to go to the grocery store, she is able to do some laundry, and she does not need any help for her hygiene. The patient almost buckled over when MD examined her left trochanter area. Otherwise, back pain is present and moderate, on the left area. The patient states that her left hip pain is worse than her back pain. The left hip is a little sensitive when trying to attempt the straight leg raise test.
IW was diagnosed with chronic low back and left leg symptoms and left trochanteric bursitis.
She was given a prescription for Norco 10/325 one tablet every 6 hours #120 with 0 refills, and a second prescription for Norco 10/325 mg 1 tablet every 6 hours #120 with note "Not To Fill Until 10/05/2016" were given. She was advised to apply some ice and heat to the inflamed swollen area of the left bursa.
The patient stated that she would like to have the trochanteric bursitis treated at Kaiser, so she is going to go to primary care doctor to request a cortisone injection because she is not able to take anti-inflammatories since she had stomach bypass. She does not take any nonsteroidal anti-inflammatory drugs (NSAIDS) either, so she is very limited on how she can control the pain.
She will follow-up in 2 months.
