After the approved “best practice” guideline was introduced to the medical staff, a blood utilization dashboard was developed that helps responsible clinical managers identify at the physician level when transfusion orders are placed contrary to the guideline. Having this information available enables the clinical manager to address the issue on a unit or with the individual physician involved. Use of this dashboard has resulted in significant reduction in the variation in transfusion ordering practices among the medical staff and a significant reduction in blood utilization, which equates to a significant reduction in costs of maintaining the blood supply, and an improvement in patient outcomes.…

A recent disaster in Nicaragua produced scores of injured persons and the need for fresh blood. The British maintain an adequate blood supply by relying wholly on voluntary donations. Because of the necessity of blood for survival, the British have resisted placing monetary value on that which has been freely given. The American system, in contrast, relies both on voluntary donation and a commercial system in which blood “is bought and sold like any other commodity” (Shaw, 2005, p. 75). Economist Richard Titmuss evaluated both systems and concluded that the British system is superior in terms of both “economic and administrative efficiency” (Shaw, p. 75). The American system suffers from high costs, frequent shortages, and an inequitable burden placed on those whose health conditions require frequent transfusions. Additionally, when blood becomes a commodity, the purity of the supply often suffers as…

NRCS had a manual system using paper cards to recruit BDs, collect/keep blood donor records and disseminate results to BDs who are scattered throughout the country. The paper card system (PCS) used to specifically capture personal data and medical history of the BDs. This information would be used in identifying/locating existing BDs, carrying out pre-donation counseling and taking blood results. Unauthorized persons however, easily accessed the paper system and hence making it impossible to keep secrecy and confidentiality expected of medical records. The security of the medical records was also not inadequate as any person could easily access them. Lukande (2003), states that such a system is time consuming, prone to errors of entry and analysis resulting from the fatigue of the users. The PCS at NRCS had lead to accumulation of physical paper cards due to increasing number of blood donors, a situation that frustrated the…
